    In this valuable role, the Clinical Unregulated Care Provider Supervisor is accountable for the supervision of PSW's and HSW's who are caring and making a difference for clients within the community. The Clinical UCP Supervisor will have the expertiseto assess, implement and evaluate a wide range of clinical care interventions to ensure the delivery of safe, quality, and ethical care.

  • What you'll do:

  • Supervise and support PSW's and HSW's to ensure practices comply with all legislated standards of practice as well as company policies and procedures.
  • Respond to and advise/resolve concerns from community staff, patients/families, and other team members.
  • Provide ongoing leadership through regular huddles, face to face meetings and one-on-ones.
  • Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.
  • Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
  • Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals.
  • Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.
  • Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.
  • What you bring:

  • Current RN or RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions
  • Minimum of 5 years' experience as a practicing Registered Nurse or Registered Practical Nurse
  • Comprehensive patient health assessment skills are required
  • Possess an understanding of the principles of community nursing practice
  • Time Type: Full time
