- Supervise and support PSW's and HSW's to ensure practices comply with all legislated standards of practice as well as company policies and procedures.
- Respond to and advise/resolve concerns from community staff, patients/families, and other team members.
- Provide ongoing leadership through regular huddles, face to face meetings and one-on-ones.
- Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.
- Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
- Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals.
- Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.
- Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.
- Current RN or RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions
- Minimum of 5 years' experience as a practicing Registered Nurse or Registered Practical Nurse
- Comprehensive patient health assessment skills are required
