    Bayshore Home Care Solutions, Hamilton Branch seeks a motivated Home Support Manager.

    Location: Greater Hamilton Area
    Nurse Level: Registered Practical Nurse (RPN)
    Salary range: $60 – 65k per annum + Health & Dental Benefits, paid vacation, matching RRSP
    Work Environment: Community based role: Daily visits to client homes to interact with clients, family members and Bayshore team members. Minimal office hours. Daily travel requirement.
    On Call Rotation: 2 – 4 times per month

    The Work You Will Do

    The Home Support Manager, is responsible for the delivery of care and service to clients and ensures compliance with internal and external standards of practice. They coordinate clinical education and in-service activities as requested. As aHome Support Manager, you will work to support and lead your team to meet the diverse needs of the clients and caregivers you serve in your community:

  • Assess or arrange for the initial and ongoing assessment of clients requiring licensed and unlicensed care and service as appropriate.
  • Develop and/or ensure that care plans are available for clients as appropriate; visit clients as required and update care plans as appropriate.
  • Supervise and support unlicensed employees to ensure compliance with all federal and provincial legislated standards of practice as well as company policies and procedures.
  • Lead or assist with the integration of client satisfaction initiatives; ensure that all referred client concerns and complaints are satisfactorily addressed and documented in a timely manner.
  • Assist the Client Service Coordinator in the coordination of services.
  • Coordinate the in-service education program for Field employees and maintain appropriate records.
  • Participate in business development as appropriate; assist with the development of contractual agreements for client care and service as requested.
  • Promote Bayshore through participation in local healthcare or business committees as well as community events.
    Complete other tasks as requested.
  • Annual Clinical Management Revisits with client/family to reassess and ensure that care plan is current and accurate for client needs and continued evaluation of client environmental safety.
  • Liaise with Funder as required to address any concerns about client needs or need for addition supports in the community.
  • Participate in community-based PPE drops in the community to support filed staff needs and offer additional opportunity for face-to-face interaction/opportunities for education.
  • Participate in Employee engagement and support visits, with addition community support as required for field staff.
  • Facilitate Transfer of Skills for unregulated staff for individual care plan needs for clients with other service providers ensuring staff competency with skills required for client care.
  • Job Qualification

    Education
    Graduate from an accredited School of Nursing at the RPN level.

    Current registration with the provincial regulating body.

    Current CPR certification required.

    Experience
    At least three (3) years of recent experience with progressive responsibilities as a Nursing Supervisor, preferably in a community setting.

    Other Requirements

    Exceptional interpersonal skills and ability to handle difficult situations in an objective consistent format; ability to work independently and effectively lead a team.

    Excellent report writing and communication skills with both Field and Administrative employees.

    Valid driver's license,, auto insurance and reliable vehicle

    Solid knowledge of the principles, practices, and methods of community nursing and service delivery to clients as well as program development, implementation and evaluation.
