    Overview

    CarePartners is embarking on a new program in partnership with the Home and Community Support Services (HCCSS) Central East and we are looking for community-based support workers (CSWs) to work within the High Intensity Supports at Home (HISH) program. These skilled CSWs will work with patients and their families to support care while patients transition through the healthcare system. The HISH program focuses in Wrap-Around Care so that all team members (including Nursing Therapy, Personal Support, Social Work and Community Support Services, etc.) are working cohesively to meet the needs of the patient. These CSWs will receive additional training and certification in Gentle Persuasive Approach (GPA).

    In this exciting staffing model, HISH CSWs will have consistent hours of work, week over week and have the opportunity to be a respected member of a larger healthcare team striving to provide high quality patient care and positive transition experience for both the patient and their caregivers.

    The hours of work will be 7, 12-hour shifts within a 2-week rotation inclusive of weekends:

    Two-week Rotation

    Mon/Tues/Sat/Sun
    hrs

    Wed/Thurs/Fri
    hrs

    This is an equivalent of seventy-seven (77) hours per bi-weekly pay period

    We are looking for Community Support Workers (CSWs) to help us bring wrap-around patient-centered care to the Port Hope/Cobourg/Colborne area. Are you someone we should meet?

    What The Role Involves

    Contributing as a valued member of the interdisciplinary care team

    Assisting with daily living activities, such as personal care (rise and retire), assistance with household tasks, respite care, medication assistance, special functions

    Promoting a safe environment by demonstrating proper body mechanics, safe transfer techniques, and home safety

    Providing cognitive, social and/or emotional support to the patient

    Preparing nutritious, varied and economical meals and snacks, including special diets, as determined by the care plan, using proper food preparation, storage, and hygiene practices

    Providing patient status reports and participate in patient/family care planning meetings

    A strong desire and commitment to making a difference in the lives of their patients

    Exceptional interpersonal skills

    Ability to work both independently and as part of a team

    Experience working with patients with significant and complex care needs such as dementia and mental health

    A valid driver's license and vehicle for which you are insured

    Community Support Worker (e.g.) Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or enrollment in a Nursing program) education, with preference given to those with a PSW certificate and/or equivalency

    To protect our patients, we require current vulnerable sector check screening, and a clear background check
