Job Summary

We are hiring a Full time NSWOC in Huron County to work M-F days in clinic and community.

As a member of the Wound Ostomy Continence (WOC) Team in Huron County provides specialized holistic assessment and management as an interprofessional team member to meet the needs of individuals/families with ostomies acute and chronic wounds urinary and fecal continence concerns.

Clinical Support Consultation Perform detailed holistic assessment of clients with wound ostomy concern formulate treatment recommendations in collaboration with the interprofessional team develop a plan care based on holistic assessment for clients who require wound ostomy care Evaluate plan care involve Wound Ostomy Resource Nurse WORN K2P Wound Care Resource Nurses K2P WC RN provide education mentorship experiences build wound ostomy knowledge capacity within SE Health Provide consultation care providers within organization Facilitate dissemination wound ostomy knowledge Coordinates/collaborates interprofessional team across continuum care reduce duplication services client anxiety cost Program Development Leadership Supports quality health care teaching mentoring program development quality monitoring Optimizes client outcomes review Quality Improvement QI reports prioritize need Identify nursing trends issues related specialty wound ostomy management Assist development information tools educational initiatives care providers Remain updated current evidence related best practices Promote participate research-based practice Identify potential research initiatives Promote participate research projects Submit case studies publication Participate conferences poster presentations oral abstracts Support WORNs K2P WC RNs development posters case studies conference presentation Collaborate local Home Community Care Support Services HCCSS initiatives/working groups required Requirements Must completed Nurses Specialized in Wound Ostomy & Continence Canada NSWOCC , international certification will considered if approved by NSWOCC Current active membership NSWOC Canada preferred Must RN current certificate competence College Nurses Ontario Flexibility commitment advancing wound ostomycare SE Health required CNA certification credential preferred Willingness further education programs/courses remain current Adoption SE model delivery through mentorshipSE model adoption requirement vehicle valid driver license willingness travel Excellent communication teaching skills comprehensive knowledge community healthcare demonstrated teaching expertise preparing effective tools methods ability use computer word processing power point email source information internet
