Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support and home care services, covering the whole Greater Toronto Area.


With a staff of 500 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges.

3,000 of whom are "home-bound" and frail.

We are currently seeking a
Program Manager to join our Home Care team.

Responsibilities:


  • Provide overall direction, control, and management for the effective and efficient operation of community and home care services delivered to various geographic communities in the GTA
  • Lead a team of operations professionals, staff and Personal Support Workers in delivering coordinated quality community health care and support to clients and their caregivers
  • Manage daytoday activities, monitor the efficacy of service delivery and utilization of resources and make recommendations for innovative models of care
  • Develop, implement and interpret organizational policy, procedures and standards to guide departmental operations
  • Responsible for the development and management of all levels of Home Care staff including hiring, training, coaching, mentoring, supervision, performance management and conflict resolution
  • Monitor team performance and activity including quality of care and documentation through ongoing program supervision, program evaluation (i.e., participate in community assessments, identify service gaps, make recommendations and develop program priorities
  • Ensure timely compilation and completion of periodic statistics and program activity reports to the Program Director and regional HCCSS
  • Build and maintain strong relationships with both internal and external stakeholders (e.g. HCCSS, VHA, etc.) for the delivery of highquality, client/familycentred and continuum of care
  • Ensure process is in place for frontline staff and care coordinators to identify and address client safety concerns and participate in ongoing internal and external meetings to upkeep the safety standards for clients as well as to the entire team
  • Responsible for accident/incident reporting, investigation, identifying root cause and recommending corrective actions

Qualifications:


  • Bachelor's degree in Nursing / Psychology / Gerontology / Social Work or equivalent
  • Additional credential is considered an asset
  • Minimum 3 years of managerial experience with progressive leadership responsibilities, preferably in a social services or community setting
  • Superior time management skills, multitasking skills, and the ability to prioritize tasks with mínimal supervision
  • Strong clinical assessment skills, with knowledge of the principles and practices of home health care, community resources, and service delivery to clients
  • Excellent communication and interpersonal skills with internal team members and external contacts
  • A welldefined sense of diplomacy, including high degree of judgement, conflict resolution and people management skills
  • Proficient computer skills in Microsoft Office, understanding of InterRAI Cha and AlayaCare is an asset
  • Flexible to change in work assignments and schedule (including occasional evenings and weekends)
  • Proficiency in English and a second language reflecting the community such as Cantonese or Mandarin is an asset
  • Valid Ontario G driver's license and access to a reliable vehicle for business purposes is an asset
  • Valid police vulnerable sector check completed within one year
