PURPOSE:

Reporting to the Director of Quality Improvements, the Minimum Data Set (MDS) Lead is accountable for collecting, analyzing and reporting Resident Assessment Instrument (RAI)/ MDS data.

The MDS Lead works collaboratively with the Clinical Leads, Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Occupational Therapist, Physiotherapist and Clinical Dietitian, to provide orientation, education and ongoing RAI training to ensure that care staff have the skills required to collect and record MDS data.

The MDS Lead ensures that all residents have completed current and accessible assessments and that resident care plans are completed.

The MDS Lead acts as a role model and ambassador for the mission and values of LCC.


ACCOUNTABILITIES AND RESPONSIBILITIES:

Leadership:


  • Leads the development of a culture of resident centered care and service, respect, collaboration, quality improvement, resourcefulness and outstanding customer service.

  • Maintains accountability for daytoday operations of area(s) of responsibility.
  • Communicates effectively with staff and ensures opportunities for staff engagement are conducted in a manner that upholds and supports a just culture.
  • Collaborates with internal and external stakeholders (AHS, Alberta Health) to ensure that the facility provides the most reliable MDS data.
  • Leads or contributes to the establishment of service performance expectations and identify areas/programs and sitespecific goals.
  • Contributes to Continuing Care Health Services Standards compliance and audit function.
  • Able to negotiate skillfully and coach mentor/staff.
  • Copes effectively with risk, change and uncertainty.
  • Maintains a high level of confidentiality of personal and medical information of residents, families and staff.

2. Areas of Responsibility

Staff Education:


  • Orientates all care staff to the RAI/MDS computerized data entry system and evaluates the effectiveness collectively and individually for staff.
  • Arranges for new care staff to attend regional MDS training.
  • Provides coaching and additional education for staff as necessary to ensure that staff are competent.
  • Identifies staff performance issues and communicates these to the Clinical Leads for followup.
  • Is visible throughout the site providing one to one coaching, mentoring, acting as a resource for staff.
  • Ensures that all staff entering data into the MDS system have yearly competency testing.

2B. Collects, analyses and reports all MDS data.

  • Works with the Director of Quality Improvement to develop processes that ensure data accuracy, efficient data collection, assessment completion and data utilization.
  • Applies the RAI/MDS data to the assessment, planning and implementation of the residents' care.
  • Adheres to established protocols, practices and standards related to MDS.
  • Works collaboratively with all members of the care team to ensure MDS data entry including all assessments and care plans is complete and timely.
  • Reviews completed data entered by care staff for accuracy.
  • Maintains statistical records and reports.
  • Reviews, prepares and submits RAI data quarterly to ACCIS.
  • Analyzes and interprets MDS data and identifies issues in order to improve data collection and reporting.
  • Regularly provides information related to MDS to the Site Leader and members of the Leadership Team, identifies any issues and concerns and provides a suggested plan to address the concerns.
  • Facilitates the use of the RAI/MDS data in clinical and operational decision making.
  • Reviews reports arising from the collected data related to medication management, resident falls, wound management; participates in the development of action plans to address concerns and completes activities related to staff education arising from the action plans.
  • Provides support to the site Quality Improvement (QI) Committee by providing data related to the QI Committee's performance indicators.
