Community Support Coordinator Position Description
At CCM, providing social support is a key aspect of our mental health support work.

We are a trusted drop-in where marginalized people of any ethnicity, sexual orientation, religion, and ability can come as the first point of contact to receive other services that support mental well-being.


The Community Support Coordinator (CSC) will develop our community support program by strengthening our individual supports and by commencing group wellness supports.

They will assist clients to access social and psychological supports by identifying individual needs and referring our clients to broader social and health services.

They will provide service navigation, referrals, and accompaniment to critical medical, dental, and social services appointments, offering emotional support to clients and advocating on their behalf.

The CSC will also facilitate wellness groups, helping reduce social isolation and improve well-being.


Position Details

Responsibilities of the CSC_

  • Conduct outreach to social service and health partners in Ottawa to enhance our working relationships and referral services.
  • Identify social, economic, recreational, medical, vocational, and educational services in the community that will meet our client's needs.
  • Develop a quick resource reference guide for our team to use in navigating local supports.
  • Be available to respond to in the moment questions and needs during dropin hours.
  • Conduct outreach to clients to inform them about available services, and help with system navigation, referrals, and accompaniment to social and health services as appropriate.
  • Identify and train volunteers to assist with accompaniment to appointments.
  • Provide emergency assistance as required and appropriate (i.e. supplies, referrals to other services).
  • Use judgement in determining when to establish an ongoing support relationship versus providing a onetime referral.
  • Understand and promote the rights and responsibilities of individuals. Empower and treat all individuals with respect and dignity. Ensure communication is respectful, objective, and unbiased.
  • Plan, advertise, and facilitate social groups for clients at the dropin to reduce isolation.
  • Ensure completion of administrative work and maintain records of support interventions.

  • Ability to be empathetic, personable, compassionate and to read people and situations effectively.
  • Awareness of and experience working with clients experiencing addiction and mental health issues.
  • Hospitable to and open to people of various Christian backgrounds as well to those of other religious traditions, or no religious tradition.
  • Must be comfortable working in a challenging environment that has the potential for confrontation and/or behavioral issues.
  • Must be able to handle personal/sensitive issues and information shared by clients and demonstrate a nonjudgmental attitude.

Experience, Skills, and Capabilities_

  • Degree in Social Work (BSW); Social Service Work; or related health discipline or equivalent combination of education and relevant experience.
  • Proven ability to positively work with a team and independently.
  • Knowledge of social, economic and health concerns relevant to urban, marginalized, and lowincome persons.
  • Excellent communication skills, able to bridge gaps between clients and service providers.
  • Knowledge of the mental health and/or addictions systems, local community resources and service navigation.
  • Assessment and intervention skills with demonstrated ability working with clients in crisis, including deescalation skills.
  • Trauma informed approach to care.
  • Proficiency with Microsoft Office tools, particularly Excel, and Word.
  • Clear Police background check for working with a vulnerable population.
  • Valid driver's license, insurance and use of a personally owned vehicle is an asset.

CCM offers a place of welcome, stability, safety, and help in Lowertown Ottawa for diverse individuals marginalized by society through a daily drop-in, support with resource navigation, and spiritual care.

The daily drop-in centre serves lunch, and hot drinks and provides clothing essentials for those in need.

Job Type:
Fixed term contract

Contract length: 14 months


Salary:
$24.00-$32.00 per hour
