Outreach Support Specialist

The position of Outreach Support Specialist is a crucial role in providing residential and outreach services to individuals experiencing mental health issues and/or developmental disabilities.

This specialist will work closely with a dynamic team to ensure that the physical, emotional, social, and medical needs of residents are met. They will assist clients with activities of daily living such as feeding, lifts, transfers, hygiene, grooming, and toileting.

Duties And Responsibilities:

• Provide residential support by ensuring client safety and well-being through appropriate means of intervention and assistance


• Develop care plans for each client addressing their psycho-social-emotional needs


• Liaise with Case Management Team to monitor progress towards meeting goals outlined in the care plan


• Create opportunities for skill-building within areas such as parenting skills anger management self-management techniques recreational socialization behaviors etc

In collaboration with the community Connective provides access programming which exists at home allows participants develop life independently reside own apartment perform household chores prepare
