Description
Key responsibilities include, but are not limited to the following:

  • Participation in the development, assessment, planning, implementation and evaluation of the program
  • Ensuring proper documentation and ongoing evaluation as per regulation and Facility policy.
  • Provisions of services in accordance with the established Residents' expectations and changing needs and the established standards.
  • Consulting with Director of Care in management of the program
  • Data collections, audits and analysis of performance indicators
  • Chairing Committee meetings, completion of minutes of meeting and distribution of minutes
  • Completion of annual interdisciplinary Program evaluation and establishing goals and objective for improvement of systems and processes
  • Functioning as a member of the nursing management team with duties as assigned which may be outside of the regular scope of duties
  • Other duties as assigned by Director of Care and or Assistant Director of Care
