We are officially starting our recruitment for the Black, African and Caribbean Health and Social Services Hub
Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) is partnering with LAMP Community Health Centre (LAMP CHC) and Roots Community Services Inc (RootsCS) to design, develop and implement an integrated health and social services hub with and for the Black, African and Caribbean (BAC) communities in Peel Region.
The hub will provide primary health, mental health, addictions and social services programs that will holistically address the physical, mental, social and cultural needs of BAC communities, and seek to eliminate disparities and inequities.

We are looking for an Addictions Support Worker
to join our team
The Addictions Support Worker will report to the Program Manager, BAC Multidisciplinary Team and will provide clients with a comprehensive care plan supporting both addictions and mental health needs using a stepped care approach**.
The Addictions Worker will conduct comprehensive assessments, provide client centered stagewise brief interventions and case management using a harm reduction model.
This position will work closely with other professionals through the provision of addictions expertise and linkages between mental health and primary health care.
- **Stepped Care approach is a system of delivering and monitoring mental health and addictions treatment so that the most effective, yet least resource-intensive treatment, is delivered first, only "stepping up" to intensive/ specialist services as required and depending on the level of client distress or need._Status:
Permanent full-time, 37.5 hours per week, evening hours and weekend may be required

This position is based out of Brampton.
Although we operate with a flexible workplace policy, staff are required to have office and/or community presence as per program and client needs.

What you will do

Conduct comprehensive standardized addictions assessments including developing Crisis Plans

Provide short-term counselling and case management support to ensure progress toward client goals

Facilitate groups as part of CMHA PD's multi-disciplinary team including the creation of new groups as needed and act as a back-up facilitator

Regularly assess risk and respond with mitigation strategies
- Provide
education on substance use and wellness to clients and their families/other supports - Take a leadership role to ensure the provision of substance use interventions for clients including motivational counseling, harm reduction, and abstinence using the stages of change model
Refer and link clients to community resources and groups that support recovery goals
- Manage a caseload including regular reassessments, updating risk levels and caseload reviews
- Participate in
regular staff meetings and care planning reviews to provide
addictions expertise for client care within multidisciplinary team
