PRIMARY HEALTH CARE

COMMUNITY FAMILY PRACTICE PHYSICIAN

One Year Contract Part-Time (0.6 FTE, 21 hours per week)

Reporting To**:
Manager Primary Care Services


Purpose:
The Physician will provide comprehensive primary health care for individuals and families living in the
Scarborough community with an emphasis on service delivery for marginalized and at risk community members.

Physicians work in an interdisciplinary team that includes other primary care physicians, nurses, nurse practitioners, social workers, chiropodists, physiotherapist, dieticians and others.


Key areas of responsibility:

  • Provide comprehensive family and client centered primary health care services within his/her full scope of practice taking into account the social and economic determinants of health
  • Provides primary care indirectly by working interactively with nurse practitioners, providing consultation to nurses or other staff, and by developing medical or clinical directive/processes.
  • Advocates for client/families to reduce barriers to accessing health care and other services in the community.
  • Adhere to standards of best practice and practicing with a "shared care model"
  • Willingness to participate in Health Centre health promotion/outreach initiatives
  • Participate in case conferences with other providers and staff to ensure coordination of client services
  • Demonstrated ability to work in team setting delivering services to marginalized individuals living with chronic illness
  • Perform other duties as assigned that are within the scope of the job.
Scope and Accountability
The Physician will function within their full scope of practice in accordance to College standards. The
Physician is accountable to the agencies program Vice President/Manager.

Educational and/or Professional Qualifications:

  • Graduate medical degree from a recognized university
  • Registration and in good standing with the Ontario College of Physicians and Surgeons
  • Registration with CMPA
  • CCFP Certification or equivalent practical experience
  • Willingness to maintain CME credentials as required.

Level of Experience:

  • 35 years medical practice in an interprofessional /CHC/FHT model of care considered an asset.
  • Willingness to work within a collaborative model of care.
Skills and Attributes

  • Strong communication, verbal, listening and writing skills
  • Familiarity or willingness to practice with electronic health record documentation environment
  • Collaborates well with others, promotes cooperation and teamwork.
  • Sensitivity to and awareness of cultural, racial, and socioeconomic diversity within the community
  • Respect and adherence to agency policies, mission, vision and values philosophy of care
  • Strong research and planning skills
  • Willingness to participate in professional development opportunities and preceptor medical/nurse practitioner students
  • Capacity to maintain professional links with other agencies, colleges and universities
  • Valid Driver's license to drive in Ontario and access to a vehicle would be considered an asset
  • Oral and/or written fluency in other languages relevant to the community the program serves is a definite asset.
Significant Working Conditions

  • Flexibility in hours of work to provide extended hours of service including evening and weekend
  • Provide on call support for Nurse Practitioners.

$100.00-$200.00 per hour
