Job Summary and Requirements


This competition will be used to fill one full time term position working 75 hours bi-weekly until December 31, 2023.

Temporary assignments may also be considered for current Yukon Government employees who have permission from their supervisor to apply. This competition may also be used to fill permanent, auxiliary on-call or positions of a similar nature.

Mental Wellness and Substance Use Services provides a range of mental wellness services to Yukoners. This position serves specific populations including individuals who live with bipolar, schizophrenia or other psychosis related mental health disorders.

Reporting to the Supervisor of the Psychiatric Outreach Program you will be responsible for assessment, treatment plan, care coordination and case management for clients with serious and persistent mental illnesses.

Mental status assessments with clients with psychotic symptoms are an important aspect of the role.


The Psychiatric Outreach Program Social Worker is also responsible for providing basic psychosocial interventions and treatment with these individuals, including motivational interviewing and basic CBT techniques.

This position is supervised by a clinician who provides clinical supervision and regular and appropriate engagement in regular clinical supervision is an expected part of providing these interventions and performance in the position.


You will also be expected to collaborate closely with mental health nurses, mental health support workers, psychiatrists, pharmacists and other health care and service providers.

You will be required to coordinate medical and psychiatric care with these clients.

An important component of this role is working as part of an interdisciplinary team, supporting clients to discuss medications with appropriate healthcare providers and problem-solving psycho-social concerns related to their medical/psychiatric health.

You will responsible for providing psycho-education to clients, families and other supports as well as facilitating psycho-educational groups.


We are interested in individuals who are passionate and enthusiastic about working with clients with bipolar, schizophrenia and other psychotic disorders.

Essential Qualifications

Bachelor of Social Work or recognized equivalent degree in health sciences or social sciences;
Considerable experience working with individuals with bipolar, schizophrenia or other psychosis -related mental health disorders.
Experience providing assessment, treatment planning, and case management to individuals with severe and persistent mental illness;
Experience providing individual psycho-education and/or delivering psycho-educational programs;
Training and experience using Motivational Interviewing and Basic CBT with clients in this population


Assets:

Experience working in a healthcare setting with medical professionals.

Either in an inpatient or outpatient setting (examples include - hospitals, tertiary care mental health facilities, community mental health treatment teams, community health centres).

Experience working in an environment informed by harm reduction principles.

Desired Knowledge, Skills and Suitability

Ability to provide psycho-social interventions including assessment, treatment planning and delivery, case management pertaining to psychotic and concurrent disorders (mental health conditions and problematic substance use)
Ability to plan, organize and manage a diverse case load;
Ability to work both independently and as part of a collaborative multidisciplinary team.
Able to show that your practice is informed by harm reduction and trauma-informed principles
Sound ethical decision making, excellent communication and conflict resolution skills.
Excellent oral and written clinical communication;
Excellent ability to initiate relationships with clients, their families and other health professionals;
Excellent analytical, problem solving and organizational skills;
Commitment to a client-centred, recovery-oriented approach.
