Right at Home Canada In-Home Health Care is part of an international family of over 600 Care Offices around the globe, and 54 Canadian offices in 8 provinces.


We provide a full suite of in-home supportive services to older adults and differently-abled younger adults who want to continue to live independently, wherever home is for them.

Right at Home Canada is currently looking for a Care Planner in Edmonton, Alberta.


This position could be part-time to start, or full time accompanied by a part-time caregiving role or part time community engagement role to create full-time position.


As a Right at Home Care Planner, you are a pillar in our Right at Home family and a crucial member of the Right at Home Care Team.

You are the most responsible person to our client families, in all aspects of a family's care; client assessments, care plan design, care service delivery oversight, and the overall client family experience.

In addition, you are the manager and motivator of your caregiver teams.


Our ideal Care Planner:


  • A positive, confident, warm, and approachable person who builds immediate trust and rapport
  • A regulated or experienced health care provider (nurse, occupational therapist, physiotherapist, etc), case manager, social worker or experienced PSW
- extensive home care/care planning experience

  • Knowledge of community resources and roles of healthcare professionals
  • Knowledge of healthcare organizations and funding models used in community
  • Excellent communication and relationshipdevelopment skills
  • A caring, compassionate person committed to enhancing the lives of those we serve
  • A resourceful, evenkeeled, multitasking individual who loves a team environment
  • Someone passionate about quality and having a meaningful impact on a family's life
  • Have a "nothing is too big or too small to ask" attitude

In this role you will:


  • Spearhead the assessment, development, and delivery of our client's individualized care plans.
  • Oversee the quality of support provided in the home
  • Network in the community as a proud and passionate advocate for the Right at Home Canada brand
  • Work closely with Right at Home client families, care coordinators, and caregivers on a daily basis
  • Work closely with client's medical professionals to ensure smooth delivery of support
  • Participate in the intake, hiring, scheduling, and supervision of caregiving staff and nurses
  • Engage directly with the Right at Home management team and other staff as an integral part of the company's goal to continue to be Canada's leader in the delivery of private home health care services
  • When not Care Planning, assist in the community awareness of Right at Home through professional and community engagement
  • Work independently from a combination of locations including your own home office or Right at Home office
While there is a typical "set" work week, this is also a self-managed role. Some client assessments during evenings and weekends may be required, with the successful applicant managing their own schedule.
