    Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families: Reporting to the Executive Director, this position is accountable for strategic leadership and program development of the Fraser Health Rehabilitation Network with a primary focus on inpatient and outpatient rehabilitation services across Fraser Health.

    Accountable to the Provincial Health Services Authority (PHSA) Health Improvement Networks (e.g.

    Stroke Services BC), which directly reports to the Ministry of Health (MOH) on Provincial mandates/initiatives and projects related to Rehabilitation Services across Fraser Health.

    The mandate involves a focus on optimizing inpatient and outpatient rehabilitation services; enhanced integration of services across sites and coordination with other rehabilitation centres in BC.

    This role seeks to improve communication within/between sites related to rehabilitation services, and timely and effective reporting to provincial and national bodies, including PHSA Health Improvement Networks (e.g. Stroke Services BC) to the Ministry of Health, and the Fraser Health Strategic Plan.

    This role also serves as the organizational lead and contact to the Canadian Institute for Health Information (CIHI) including input to the National Rehabilitation Reporting System (NRS), membership on provincial/national advisory committees as appropriate and providing input to Ministry of Health reports from the NRS on behalf of Fraser Health.

    Leadership responsibility aligns with Rehabilitation Network mandate of Quality Improvement, Standardization and Health Human Resource planning, including comprehensive reviews of practices, procedures and systems leading to enhanced integration of services, better service delivery to patients, improved patient satisfaction, development of new programs and the delivery of cost and operational efficiencies for Fraser Health and the health care system.

    Participates in the development of a provincial database used to track demand and inform quality improvement and performance outcome targets.

    Works in close collaboration with FH leadership and various Health Improvement Networks in BC and the Canadian Institute for Health Information; provides strategic leadership and accountability to increase rehabilitation access across FH through the development and implementation of a variety of cardiac initiatives; the strengthening of "Tele-rehab first" philosophy/capability in specified areas of new development.

    The role requires effective collaboration, consultation, liaison and negotiation with senior leaders and stakeholder networks across FH, based on a strong capacity for influence.

    Liaises with external agencies such as the MOH and Cardiac Services BC for planning and reporting purposes.

    Responsibilities Accountable to lead the development of the FH Rehabilitation Network strategic plan including to lead the operational development and implementation of Fraser Health and Health Industry Number (HIN) strategic initiatives related to rehabilitation across Fraser Health.

    Provides strategic leadership, oversight and accountability to increase rehabilitation access to inpatient and outpatient rehabilitation in acute care sites that provide these services across Fraser Health in alignment with provincial initiatives.

    Develops and implements rehabilitation initiatives employing cornerstone initiatives such as implementation of new technology and procedures and capacity planning & expansion.

    Reviews, evaluates, develops and implements new or revised initiatives to inpatient & outpatient rehabilitation capacity through integration of research at sites.

    Collaborates with Health Authority Rehabilitation Leads, Directors of relevant provincial Health Improvement Networks, Provincial Lead for the CIHI NRS and Rehabilitation CNS on informing and developing guidelines and interpretation documentation.

    Facilitates clinical leadership within major redevelopment projects; and develops and implements education plans for sites.

    Based on the strategic goals of Fraser Health, consults with other health authority rehabilitation centres and provincial health improvement networks to prioritize quality and renewal initiatives/projects, define project scope and lead the development and implementation of policies, guidelines, measures of evaluation and reporting systems.

    Directs and leads the development of action plans to meet designated goals within mandated timeframes.

    Provides direction, oversight and coordination of project resources including advising and supporting project work groups/committees, advancing priorities, coordinating resources, facilitating/promoting collaboration and commitment of key stakeholders; and successful integration of project quality improvement findings and recommendations.

    Collaborates and works closely with leadership and professional/technical teams to ensure the successful implementation of rehabilitation priorities including new development and redevelopment projects; waitlist management, and increased capacity.

    Consults, advises and collaborates with clinical and site leaders at major redevelopment sites regarding expansion, improvement of inpatient/outpatient rehabilitation capacity.

    Provides best practice expertise through strategic and operational planning, research & evaluation, financial assessment, forecasting/model development, feasibility studies, and business case development associated with reviews of practices, procedures and systems.

    Ensures the needs of program areas are appropriately represented in the various initiatives related to Rehabilitation.

    Advises on new alternatives to delivery practices which facilitate/support the effective and efficient utilization of resources in the provision of health care service delivery.

    Accountable for operational management of specific initiatives from concept through to implementation and post-transition ensuring adherence to project timeframes and budget.

    Liaises closely with Health Improvement Network Directors, Health Authority counterparts and CIHI regarding designated Provincial/national initiatives/projects relating to the timely development of strategies, plans, models, systems, procedures and processes related to rehabilitation.

    Establishes a collaborative framework for network leaders, teams and provincial stakeholders to participate and contribute to the development and implementation of required initiatives, including the coordination and leadership of resources from participating areas.

    Provides strategic leadership through consultation and collaboration with stakeholders to ensure all facets of the project/initiative has been vetted in order to identify issues/areas of concern, or identify risks which may impact project implementation readiness.

    Ensures newly designed and/or modified policies, processes, protocols or program structures are developed and implemented which support desired outcomes. Ensures all initiatives integrate seamlessly to support the overall goals of FH.

    Develops financial proposals, cost assessments, briefing notes and budgets; collaboration with stakeholders, engagement of appropriate resources, facilitation of project activities, progress monitoring, problem resolution, and recommendations to senior management team.

    Leads and oversees assigned resources to ensure systems, processes and needs for change are identified and assessed, and appropriate action plans developed.

    Leads the development of evaluation criteria for developed and/or ongoing projects/initiatives.

    Advises on evaluation strategies; facilitates the evaluation process; oversees the monitoring and appraisal of project outcomes as it relates to established program expectations.

    Working with project teams, develops, coordinates and provides regular reports to the ED, FH rehabilitation network leadership, relevant PHSA Health Improvement Networks and CIHI outlining project status, potential issues/obstacles, project implementation readiness; modifies initiative milestones as necessary.

    Reports to the ED and Network Leadership team on system and operational changes required for successful implementation including the status and risks.

    Plans responsive care delivery systems and services and resolves and facilitates the coordination of services within Fraser Health. Ensures appropriate reporting through Fraser Health Report Cards or other performance measurement tools as appropriate.

    Fosters effective relationships across rehabilitation services within BC, FH and the other Health Authorities involved to maintain support for all rehabilitation initiatives and activities.

    Ensures stakeholders are kept current of status; recommends and creates partnerships across Health Authorities as appropriate.


    Qualifications QUALIFICATIONS:

    Master's degree in Nursing, Allied Health, Leadership disciplines such as Business, Health Administration, Project Management supplemented by seven to ten years' related experience in a complex health care environment with demonstrated experience in inpatient rehabilitation, outpatient rehabilitation, virtual rehabilitation and/or preventative/self-management functional strategies.

    In addition, demonstrated operational and/or project leadership in strategic and operational change/re-design in an environment serving stakeholder groups encompassing the public, organizational network leaders, provincial representatives, physicians or an equivalent combination of education, training and experience, is required.

    Demonstrated ability to liaise and negotiate with network leaders, provincial representatives and a range of shareholders including the public and physicians.

    Knowledge of current Provincial Health Care policies. Excellent written and verbal communication skills. Ability to operate related equipment. Physical ability to perform the duties of the position.
