The Social Worker is a member of the inter-professional Dual Diagnosis Consultation Outreach Team serving Southeastern Ontario.

The team provides assessment, consultation, and short-term treatment for people over the age of 16 with a dual diagnosis (an intellectual disability and a diagnosed or suspected mental illness including behavioural disorders.) Utilizing a shared care model, the team works in collaboration with community partners and family physicians to reduce the impact of mental illness and improve the quality of life for adults with a dual diagnosis and their care providers.


As a member of the team, the Social Worker will utilize discipline specific skills as defined by the Ontario College of Social Workers and Social Services Workers (OCSWSSW).

The social worker will provide assessment, diagnosis, treatment and evaluation of individual and interpersonal problems through the use of social work knowledge, skills, interventions and strategies, to assist individuals, couples and families to achieve optimum psychosocial and social functioning.


Skills and Qualifications Required:


  • Master's Degree in Social Work
  • Current registration or eligibility for registration with the Ontario College of Social Workers and Social Service Workers, required
  • Annual Completion of OCSWSSW Continuing Competence Program to provide evidence of continuing competence to practice social work, according to the guidelines set by the College
  • Minimum of 3 years experience in dual diagnosis / psychiatry or similar equivalent
  • Experience working in a wide range of community settings including clients' homes
  • Proven experience in working with families and community agencies
  • Community development experience preferred
  • Experience providing case management services, preferred

Position Responsibilities:


  • Provide short term counseling for a clinical caseload
  • Review and evaluate the treatment and ongoing care plan in consultation with team psychiatrist and other team members
  • Document on client's file, psychotherapeutic interventions and assist with monitoring pharmacological interventions for both efficacy and adverse reactions, based on the standards set out by the College of Social Workers and Social Service Workers of Ontario Communicate any significant changes in client status to the appropriate members of the community health care team including the client's' family physician
  • Provide short term individual psychotherapy and lead groups adapting therapeutic modalities (CBT, DBT) to accommodate persons with a dual diagnosis
