The Work You Will Do
As an MCP, you will work to meet and mentor the diverse needs of the clients and caregivers you serve in your community by providing a wide range of services:

  • Hybrid Role: Bayshore Office, Home Office and Client Locations
  • Travel within the community will be required on a regular basis


  • Travel within BC

  • Lower Mainland (Vancouver, Burnaby, Tricities) will be required
  • Understand and support the mental health needs of clients and caregivers/nurses
  • Work with Complex client care needs within the community
  • Building and designing care plans in collaboration with other MCPs or Area Director to facilitate scope of needs for complex clients
  • Provide mentorship, supervision, performance management to caregivers/nurses while working oneonone with your clients to provide care in a facility or in their own home; deliver the highest quality home health care with compassion, excellence, and reliability
  • Provide client care by using the skills of observation, assessment, nursing diagnosis, and counseling
  • Participate in ongoing internal and/or external continuing education activities
  • Provide education and training to caregivers/nurses on an ongoing basis
  • Provide care, on occasion, including standard care services as well as specialized, client specific procedures when required
Job Qualification


Who You Are

  • You are an RN/LPN with at least three years of recent experience with progressive responsibilities as a Nursing Supervisor, preferably in a community setting
  • Solid knowledge of the principles, practices, and methods of community nursing and service delivery to clients as well as program development, implementation, and evaluation.
  • Desire to help improve the lives of those in your community through mentorship and training activities
  • Hold a current RN/LPN license in good standing within BC. Graduate of a nursing degree/diploma from an accredited School of Nursing
  • Evidence of current CPR certification
  • Ability to work independently and effectively lead a team; above average report writing and communication skills with both Field and Administrative employees.
  • Have valid driver's license and reliable vehicle for commuting
  • A clear Police Criminal Record
  • Exceptional interpersonal skills and ability to handle difficult situations in an objective consistent format
  • Must have received the full series of a COVID19 vaccine or combination of COVID19 vaccines approved by Health Canada**Bayshore is committed to a high quality and safe environment for both our staff and clients/patients.
