Role:

Develop and maintain a strong rapport with clients and their families.
Assess individual needs and create personalized care plans.
Provide emotional support and companionship to enhance the overall wellbeing of clients.
Organize and facilitate activities that promote social interaction and mental stimulation.
Maintain accurate records of client interactions and progress.
Prepare regular reports on the social and emotional wellbeing of clients.
Communicate effectively with the care team to ensure seamless support

Requirement:

Be a graduate of an accredited Social Work program or equivalent in a clinically relevant field.
Proven experience in social support, preferably in a home setting.
Excellent communication and interpersonal skills.
Compassion and empathy towards the diverse needs of clients.
Ability to work independently and as part of a collaborative team.
Knowledge of community resources and support service
Possess good communication, critical thinking, and organizational skills in a professional setting
Hold a valid work permit (if applicable)
Possess a current CPR/First Aid Certification
Will be able to provide a clean vulnerable sector screening (VSS)
Have uptodate immunization records (MMR, TDAP, Varicella, Hep B, COVID19)
Hold a valid drivers license and have access to a vehicle (considered an asset)
