Role:


  • Develop and maintain a strong rapport with clients and their families.
  • Assess individual needs and create personalized care plans.
  • Provide emotional support and companionship to enhance the overall wellbeing of clients.
  • Organize and facilitate activities that promote social interaction and mental stimulation.
  • Maintain accurate records of client interactions and progress.
  • Prepare regular reports on the social and emotional wellbeing of clients.
  • Communicate effectively with the care team to ensure seamless support

Requirement:


  • Be a graduate of an accredited Social Work program or equivalent in a clinically relevant field.
  • Proven experience in social support, preferably in a home setting.
  • Excellent communication and interpersonal skills.
  • Compassion and empathy towards the diverse needs of clients.
  • Ability to work independently and as part of a collaborative team.
  • Knowledge of community resources and support service
  • Possess good communication, critical thinking, and organizational skills in a professional setting
  • Hold a valid work permit (if applicable)
  • Possess a current CPR/First Aid Certification
  • Will be able to provide a clean vulnerable sector screening (VSS)
  • Have uptodate immunization records (MMR, TDAP, Varicella, Hep B, COVID19)
  • Hold a valid drivers license and have access to a vehicle (considered an asset)
