- Conduct initial client assessments, identification of problems, and establishment of goals and treatment plans
- Develop client-centered plans with clients, inter-professional team and other stakeholders
- Engage clients using a collaborative approach, build rapport and trust, and achieve a mutual understanding and agreement related to the services to be provided
- Maintain professional client records related to client encounters and plans
- Work collaboratively with a multi-disciplinary team consisting of government agencies, healthcare workers, and other community resources What you bring:
- Bachelor's degree in Physiotherapy
- Registered and in good standing with the College of Physiotherapists of Ontario
- Willingness to travel to and from client's residences within a designated area
- Valid driver license and access to a vehicle to travel to and from patient's residences within a geographical area Time Type: Part time
