Job summary

The Clinical Planner is accountable for onboarding new patients requiring Personal Support Services within an assigned district through the assessment implementation and evaluation of a wide range of clinical and personal care interventions.

Responsibilities

  • Perform the initial assessment of care needs for new patients and establishes the individual care plans for patient services.
  • Complete the initial safety assessment of a new patients living environment to ensure employees will be working in safe and healthy work conditions.
