Summary of Duties


Sunnybrook Health Sciences Centre has a vacancy for a Patient Safety Specialist within the Department of Quality & Patient Safety, focused on developing and implementing hospital wide initiatives that improve patient and staff safety, and patient safety culture.

Specifically, key functions of this role include the analysis of patient safety incident data, collaboration with clinical teams to develop systems to reduce the potential for errors and offer expert advice in mitigating further risk of error.

This role will report to the Manager of Quality & Patient Safety.


Specific Responsibilities include:


  • Analyse patient safety incident data/trends (together with Risk Management) to identify areas of greatest risk, and collaborate with key stakeholders to design and implement effective system improvements
  • Lead strategic patient safety priority projects for the organization in collaboration with teams
  • Provide expert consultation to various clinical groups regarding issues related to patient safety, using key patient safety principles (Incident Analysis / Root Cause Analysis, Human Factors, Systems Approach, FMEA, etc)
  • Develop patient safety policies and foster a culture of safety in consultation with others
  • Lead and participate in the Patient Safety Leadership committee that enables collaborative dialogue, decisionmaking regarding patient safety trends, and required actions
  • Contribute to the achievement and maintenance of corporate patient safety standards and Required Organizational Practices
  • Deliver education related to patient safety using multiple modalities (e.g., webbased learning; didactic sessions; simulation exercises, etc.)
  • Provide patient safety expertise to key hospital committees, where required

Qualifications/Skills:


  • Degree in healthrelated field, Quality & Patient Safety or Health Administration/Policy preferred (Masters considered an asset)
  • Regulated Health Professional with current Regulatory College Registration in good standing preferred

Incident analysis and patient safety training or certification required:

  • Project Management training / experience preferred
  • Minimum 3 years patient safety improvement experience in a clinical environment required
  • Experience in managing broad (e.g. hospitalwide) improvement projects
  • Demonstrated change management skills
  • Excellent verbal and written communication skills
  • Proven organizational, problem solving, time management, meeting facilitation and critical thinking abilities
  • Selfdirected and strong collaborative skills
  • Experience with adult learning an asset
  • Experience with MS office required
  • Experience with databases an asset
  • Experience preparing manuscripts for peerreview an asset
