    Reporting to the Clinical Service Manager, the Resource Nurse is responsible for the interdepartmental activities related to the day to day operation of the Acute Care For Elderly (ACE) Unit. The Resource Nurse collaborates with the Clinical Service Manager in the creation of an environment that encourages and supports creativity, innovation, critical thinking and a healthy/respectful workplace environment.

    The Butterfly model is a model of care that is truly patient-centred. It is an interactive model of care that involves special patient-nurse interactions to help eliminate behaviors' that can be a result of delirium and/or dementia. The core values are making the core skills of caring staff to feel, look, connect and occupy so they create a community of share, reach, relax and matter.

    In this role you will use high levels of emotional intelligence, passion for the care of those living with dementia, and the ability to relate to others to ensure transformation to an emotions-based model of care on the ACE Unit.

    Accountabilities:

    • Work collaboratively within the Butterfly model team to implement, evaluate and sustain the foundational components of the model
    • Support the Butterfly concept and model by attending education related to the model of care
    • Inspire and lead others to be a Butterfly through role modelling, coaching and through communication
    • Participate in change methodology to support and sustain the change to an emotion-based model of care
    • Incorporate your knowledge of all emotions meaning something to lead staff in developing creative and person-centred strategies to support those living with dementia to be successful and emotionally well
    • Daily staff assignments are created based on patient acuity matrix and skill mix to ensure appropriate and equitable patient workload
    • Work collaboratively with staffing office to meet daily staff needs and supports activities related to Kronos
    • Collaborates with Infection Prevention and Control, Occupational Health and Safety and Human Resources related to patient and staff safety
    • Collaborates with peer resource nurses, and program clinical nurse educator to identify and optimize learning opportunities at the unit level and support organizational initiatives
    • Responsible to facilitate patient flow throughout the organization by identifying and removing barriers to admission, transfer and discharge at the unit level in collaboration with the clinical service manager and attends daily bed utilization meetings
    • Compliance with policies and procedures associated with observations
    • Professional, patient-centred care to ensure patient safety and physiological and psychological well-being, associated with creating a therapeutic relationship and environment
    • Providing care in a compassionate and caring manner within a diverse cultural environment
    • Practicing according to the infection control and prevention measures
    • Actively participates on committees related to clinical practice (i.e. nursing advisory committee, resource nurse forum, joint occupational health and safety, unit-based council, working groups. etc.)
    • Role models program practice expectations and care pathways and Osler policies
    • Support nurses in their clinical practice to be exemplary in the areas of:
      • Safe nursing practice in all aspects of the nursing process: assessment, planning, implementation and evaluation of patient care
      • Professional, patient-centered care to ensure patient safety and physiological and psychological well-being
      • Providing care in a compassionate and caring manner within a diverse cultural environment
      • Practicing according to the infection control and prevention measures
      • Responding to emergency situations by recognizing changes in patient's health status and consulting appropriately when necessary using transfer of accountability principles
    Qualifications
    • Currently registered and in good standing with the College of Nurses of Ontario
    • BScN required
    • Minimum five years recent acute medicine experience
    • Minimum of three years recent experience as unit leader
    • Comprehensive understanding of geriatric syndromes, delirium and dementia
    • Commitment to promotion of senior friendly care using the NICHE model
    • GPA certification required
    • Geriatric Certification enrollment required
    • PIECES enrollment required
    • NICHE geriatric certificate preferred
    • A passion for leading dementia care
    • Up-to-date and evidenced base knowledge and skill in dementia care
    • Your priority is the well-being of those patients on the unit, not only tasks

    • Ability to prioritize patient care needs and make autonomous decisions
    • Excellent interpersonal, organizational and communication skills and demonstrated ability to mentor staff

    • Exemplary clinical practice within an inter-professional team to develop appropriate strategies for patient care
    • Demonstrated commitment to continuing education related to clinical practice
    • Demonstrated excellent customer service, organizational and critical thinking skills
    • Excellent attendance and work record
    • May be required to work and travel between all Osler sites

    Hours: Currently days, 8 hour shifts Monday to Friday (subject to change in accordance with operational requirements)
