    Overview

    We are currently looking for someone to join our dynamic Rehabilitation Team in the role of Registered Dietitian. We are actively expanding our therapy/rehabilitation program and are looking for professionals who want to be a part of this exciting growth. Registered Dietitians uses their clinical skills and assessment tools to perform a thorough assessment of each client. Our clients are a healthy mix of all age groups from pediatric care for children with complex needs all the way to the senior population. They then create appropriate goals in conjunction with the Home and Community Care Support Services (HCCSS) to treat the client. As a whole, our Dietitians provide exceptional care to clients in order to help clients reach their goals.

    This position covers the Huron and Perth County areas and because it is community-based, there will be travel between and within these different locations.

    What The Role Involves

    • Using your professional assessment skills to perform a holistic assessment of each patient and their unique needs
    • Utilizing information from the client, caregiver(s), the home environment, the HCCSS referral and other sources
    • Functioning as a subject matter expert in all things dietary, nutritional and hydration
    • Acting as a member of a vibrant interdisciplinary therapy team who is accountable for the therapy care it provides
    • Developing appropriate time-specific, measurable goals and outcomes in collaboration with the client and/or caregiver(s), and in keeping with the HCCSS goals for treatment
    • Being committed to quality and accuracy of care in alignment with our mission, vision and values

    What You Bring

    • Degree (or equivalent) in sciences and human nutrition, administration and humanities; from a recognized University
    • Registration with the College of Dietitians of Ontario
    • Proven computer skills, including email, word processing, work scheduling systems
    • Strong assessment skills and the ability to participate in effective treatment programs
    • Provide one-on-one therapeutic support with clients
    • Promotes independence of the client in the home and in the community
    • A reliable vehicle for some regular travel
    • Excellent English verbal and written communication skills
    • Experience in providing service in a community care setting is preferred
    • Must have a reliable vehicle and driver's license
    • Clear Background and Vulnerable Sector Check
