About us:
Thrive Counselling is a family service agency serving the Halton Region.
We provide a variety of counselling and other services to support our community and increase their well-being and safety.
We are currently launching a new service hub to assist individuals and families impacted by abuse and violence.


About the role:
This role will assist in the initial coordination of the service hub
partners and act as Project Lead on the hub development. The System


Navigator will work to establish optimal integration of the various service providers who will be supporting the hub development and operations.

Supported by both an advisory and coordinating tables, the System Navigator /Lead will work to establish MOUs, referral pathways and collaborative service relationships to ensure that victims and survivor of intimate partner violence, sexual violence and human trafficking can easily access the services they need in a seamless and coordinated environment.


Responsibilities:
Enhance the connections and pathways between various community resources to meet the needs of clients

  • Improve access to service, strengthen relationships with service providers and assess service coordination and outcomes
  • Coordinate & connect the various services/supports involved with clients
  • Collaborate with the hub partner organizations in the development of services provided
  • Liaise with the advisory and community committees working in the area of violence and abuse prevention, treatment and response with the goal of improving engagement and continuity across services
  • Assist with the development of protocols and procedures for a hub model of care

requirements:
Masters preferred or equivalent education/experience

  • Experience or knowledge of genderbased violence and communitybased resources
  • Excellent interpersonal, oral & written communication skills
  • Proven ability to work collaboratively
  • Demonstrated initiative and experience working independently with mínimal supervision

Job Type:
Fixed term contract

Contract length: 30 months


Salary:
$80,000.00 per year


Education:


  • Bachelor's Degree (preferred)

Hybrid remote in Burlington, ON L7N 2P5
