POSITION:
Service Planning Coordinator
LOCATION:
Simcoe County
PURPOSE OF POSITION
To provide children and youth with multiple and/or complex special needs and their families with a seamless and family-centered service experience.
STATEMENT OF MAJOR RESPONSIBILITIES
To provide consultation and co-ordination to children and their families by:
- Developing a strengths-based Coordinated Service Plan that addresses the service needs of the child/youth, is driven by the goals of the child/youth and family and that will support participation at home, school and in the community.
QUALIFICATIONS
Education and Experience
Post-secondary degree at a university level in one of the social or behavioral disciplines, with directly related experience (2 to 3 years) in working with children/ and families with complex needs.
Knowledge and Skill Level
- Extensive knowledge of child development and family systems theory and an understanding of the impact of a child with exceptional needs on family development.
- Ability to work independently and cooperatively as part of a team; able to manage time flexibly and effectively.
- Ability to develop trusting, cooperative relationships with clients, families, service providers and stakeholders.
- Ability to respond to complex situations and assist families in time of stress.
- Strong knowledge of family systems and strengthbased planning and interventions.
- Understanding of positive based behavioral intervention strategies.
- Knowledge of community resources, school system and awareness of relevant legislation
- Sensitivity, understanding and ability to work with a variety of value systems, beliefs and cultures.
- Capacity to work independently
- Aware of Health and Safety Rules and Regulations.
- French Language considered to be an asset.
Hourly Range:
$29.41-$33.09
