Detailed Overview:


The Social Worker works as part of a multi-disciplinary team using a collaborative approach ensuring continuity of care with community resources.

The Social Worker conducts comprehensive social work and psycho-social assessments of clients and families using a variety of modalities and in accordance with professional standards and clinical policies and practices.

The Social Worker provides direct care to clients and families in order to assess needs, support and link clients with community services and participates in the evaluation of the program to ensure maintenance of standards of care.

The Social Worker develops, coordinates and delivers inservices, presentations and psycho-educational groups.

Incumbent will also be required to provide services in Mental Health & Substance Use-CRESST (Fraser East).


Responsibilities:


  • Provides direct care to clients through formation of psychosocial assessment and treatment plan, gathering information about the client and client family with respect to social, emotional, cultural, environmental and financial situation, and utilizing a variety of treatment modalities to provide treatment; documents care plan in accordance with established standards and procedures.
  • Liaises with primary physician, team members, and other staff and referral sources regarding client care issues.
  • Liaises with appropriate health and community resources in order to maintain a strong interrelationship with the hospital and/or community resources and to provide the relevant information to facilitate service linkages as appropriate.
  • Provides consultation and education to lay and professional community members or agencies to address the needs of persons with serious and persistent mental illness.
  • Organizes and implements psycho education groups.
  • Participates in the orientation and collegial sharing of information to staff and students, as well as, education sessions for community members and/or agencies. Develops, coordinates and delivers inservices, presentations, and/or training for other health professionals on topics related to mental health issues and services by methods such as collaborating with members of the interdisciplinary team, determining relevant content, conducting literature searches and planning and coordinating seminars.
  • Facilitates clients' utilization of appropriate community services and rehabilitative programs by performing duties such as researching available services, liaising with these agencies, providing related advice and consultation to clients and providing referrals to appropriate community treatment alternatives and services.
  • Facilitates the process of admission when required by preparing the client and providing pertinent information to the Psychiatric Liaison Nurse and physicians in the Emergency Department.
  • Provides support and direction to clients by assessing their adjustment and ability to perform routine life activities, including managing money and taking medication regularly.
