  • The funding associated with the COVID19 IPAC Hubs in each LHIN is being provided to support LHINs, public health units, hospitals and hub partners to work together in a partnership that builds on these onetime investments to implement IPAC Hubs to respond to the needs for expertise and support from community based congregate living settings._
  • As part of the HNHB IPAC Satellite Hub, the Health Promoter will support Niagara's IPAC Satellite Hub/IPAC Specialists through the following:_
- _Determine scope and degree of communication with Hub Partners and similar positions (including Public Health Ontario, local public health units and other providers) and congregate living settings in defined catchment areas_
- _Determine types and amount of services provided as part of the IPAC Hub to congregate living settings through a risk-based framework to determine allocation and priority, including:_
- _Development of education and training resources/tools, use of social media, website(s) and other promotional activities;_
- _Support communities of practice through information sharing, learning and networking among IPAC leaders within congregate living settings;_
- _Support the development of IPAC programs, policies, and procedures both internally and externally;_
- _Support assessments and audits of IPAC programs and practice;_
- _Support public health partners and congregate living settings to develop outbreak management plans; and_
- _Promote the implementation of IPAC recommendations within congregate living settings._
- _Complete monthly reporting to the Lead IPAC Hub Agency for submission to the Ministry, including but not limited to:_
- _Coordination and communication activities;_
- _Service delivery (types, amount, sectors e.g. LTC)_
- _Trends and key observations_
- _Number of requests for service_

JOB SUMMARY:

Reporting to the Program Manager, the Health Promoter is an integral member of a multidisciplinary team.

The Health Promoter applies their expertise in determinants of health, health promotion and community development to support programming under the Ontario Public Health Standards and the relevant legislation such as the Health Protection and Promotion Act.

The Health Promoter identifies program priorities by reviewing existing epidemiological, behavioural and socio-demographic data, and thorough assessments of community assets and needs.

Through facilitating community mobilization, the Health Promoter builds capacity and partnerships around shared health priorities.

The Health Promoter maintains current knowledge of the literature and best practices and critically appraises research to assist with setting priorities, targeting programs and developing public health strategies and health promotion interventions.

The Health Promoter may be assigned to one of several programs in the divisions as noted above.

EDUCATION:


  • Bachelor's degree required in Health related discipline

KNOWLEDGE/EXPERIENCE:


  • Two to three years' experience in community based health promotion.
