ABOUT THE ROLE


The
Urban Tele-Mental Health Coordinator is a permanent full-time position that enables coordinated access to psychiatry at the Hospital for Sick Children (SickKids).

Provides intake coordination and liaison support with community partners and SickKids Tele-Mental Health Services.

Community engagement and outreach/promotion of Child and Youth Urban Tele-Mental Health Services to community partners across the Toronto and the GTA.

WHAT YOU WILL DO

  • Support case workers with community referrals and liaise with SickKids to book and confirm virtual psychiatric consultation appointments.
  • Record all referrals in the Access database and document the status of the referral throughout the process.
  • Increase awareness and usage of the services.
  • Develop and maintain an outreach plan to ensure that community agencies are engaged and informed of the services.
  • Ensure representation of the urban site experiences within the Tele
  • Mental Health Network.
  • Identify need for community partnership development; manage and maintain existing partnerships.
  • Ensure documentation is recorded accurately and reflected in a summary report for the Ministry of Health.
  • Support the processing of fax referrals from primary care to Help Ahead for mental health services.

THE "MUST HAVES"
The following education, professional memberships and knowledge are a requirement for this role:

  • Post-Secondary Education in a Human Services or related field.
  • Knowledgeable of client databases.
  • Ensuring confidentiality and recordkeeping.
  • Intermediate computer proficiency in order to use Microsoft Office, Outlook and other computerized systems and databases.
  • Minimum two years of administrative experience.
  • Knowledge of pulling statistics from both databases.
  • Knowledge of Microsoft Teams and Zoom.
  • Knowledge of Microsoft Office.
  • Knowledge of CYMH Services/Hospital Services.
  • Knowledge of UTMH Services through SickKids.
  • Knowledge of the Mental Health System.
  • Awareness of the role of diversity, equity, and inclusion in the workplace and within the community being served by the position.
  • An understanding of Anti-Black Racism, antioppressive/antiracism perspective in the context of the social determinants of health when working with clients and their families.
  • Ability to recognize and identify situations where incidents of and imbalance of power and privilege may occur and can result in possible negative impacts for the client.

THE 'NICE TO HAVES'

  • Trained in Conflict Resolution.
  • Trained in CPR.
  • Knowledge of specialized agencies/services that may access services.
  • Functional computer and client information system knowledge.
  • Knowledge of the cultural diversity of communities served by the organization.
  • Fluency in another language(s), including sign language.
