Term Full-Time Housing & Outreach Support Worker (Tri-Cities)


REPORTS TO:
Program Manager, Tri-Cities Winter Shelter


DESCRIPTION:


The Housing & Outreach Support Worker provides outreach and case management to individuals with past and recent issues of homelessness and to assist in housing and community living.


JOB SITE:
Tri-Cities Area


EMPLOYEE CATEGORY:
Full-Time, Term (Until March 31, 2024 with the possibility of extension)


NUMBER OF VACANCIES: 1 (one) vacancy


HOURS OF WORK:
Monday to Friday 2:00PM - 10:00 PM


SALARY RANGE:
Grid 3- HEU - Community Subsector


QUALIFICATIONS

  • Bachelors degree or diploma in Social Work, Psychology or other Social Sciences related discipline.
  • 1+ years recent related work experience with vulnerable populations.
  • Life experience may be considered in part for related experience. OR An equivalent combination of education, training and experience relevant to the level of work.
  • A valid Class 5 BC driver's license and access to personal vehicle for business related purposes.
  • First Aid and Provincial Violence Prevention certification an asset.

KEY DUTIES AND RESPONSIBILITIES

  • Being available to go out into the community on a regular basis to establish a rapport and meet with clients.
  • Picking up and transporting clients to Shelters within the community in the agency van.
  • Locating people who are in need of support services and who otherwise would not seek assistance.
  • Engaging unsheltered people to establish rapport which will assist them in locating rental units or attending appointments within the community.
  • Maintaining excellent relations with health and social services professionals such as workers with the Ministry of Social Development and Social Innovation, Canada Revenue Agency, PHS Mental Health, Burnaby Mental Health Services, BC Housing, Burnaby Substance Use Services, Burnaby RCMP and other non profit and community agencies involved in the provision of services.
  • Is an active participant and member of the Integrated case management team, and broader multidisciplinary Housing and Outreach Program.
  • Facilitating a full assessment of client's housing and support needs, using mandated assessment tools and templates.
  • Developing a client centered plan with each client for the planning and preplacement of the individual's tenancy and support requirements within the community.
  • Monitoring the areas of daily living, financial, health, social supports and resource acquisition.
  • Working in a team environment on developing housing initiatives and finding creative solutions to housing in challenging market conditions.
  • Maintaining positive relationships with landlords of private and society operated affordable housing, keeping up to date on availability of long term housing.
  • Maintaining accurate, up to date and appropriate records and ensuring all data is current and captured in relevant databases.
  • Completes and maintains current files, case notes, daily intakes and relevant client information.
  • Completes agency required staffing forms such as time sheets, leave requests and expense claims.
  • Performs other related duties as required.

Full-time, Fixed term contract

Contract length: 4 months


Salary:
$30.64 per hour

Expected hours: 40 per week
