Position Description:

***: To provide pharmaceutical care to patients within the hospital and to support the hospital's drug distribution system, mentoring Pharmacists and Pharmacy Residents. Work will be conducted in accordance with established professional standards and the hospital's policies and procedures. Acts as a medication expert both as a consultant and resource for other health care professionals and patients. To educate pharmacy and medical learners, other health care professionals and patients. To work within established professional competency standards and the hospital's policies and procedures. To ensure optimal patient care outcomes through effective identification, prevention, and resolution of drug-therapy problems and the development and implementation of a pharmacy care plan in assigned patient care area.


QUALIFICATIONS & SKILLS:


  • Bachelor of Science in Pharmacy or Pharm D from an accredited university
  • Registration with the Ontario College of Pharmacists, Part A
  • Hospital Pharmacy Residency plus 3 years experience in practice area or related practice area OR Advanced Clinical Degree plus 2 years experience in practice area or related practice area OR 5 years experience in practice area or related practice area
  • Expected to demonstrate leadership qualities within the pharmacy team with respect to assigned clinical area and/or area of expertise
  • Willingness to work collaboratively in a multidisciplinary team environment
  • Excellent oral and written communication skills
  • Must have excellent interpersonal skills
  • Effective timemanagement skills
  • Ability to perform multiple tasks and work under pressure
  • Accuracy of medication order interpretation and processing is essential

RESPONSIBILITIES:


Some responsibilities include:

  • Ensure optimal patient care outcomes through effective identification, prevention, and resolution of drugtherapy problems and the development and implementation of a pharmacy care plan
  • To mentor Pharmacists and Pharmacy Residents in assigned patient care area. To participate in the education of Pharmacy Students and Residents, as required. To act as a consultant and resource for other Pharmacotherapy Specialists and Pharmacists
  • To perform and/or supervise those tasks of drug distribution necessary to ensure the safe and accurate distribution of medications. This involves interpreting and processing medication orders accurately and effectively problemsolving drug distribution issues as they arise. Order entry is decentralized during regular working hours. To ensure the costeffectiveness of drug therapy.
  • Provide educational sessions to pharmacy staff, medical students and residents, nursing staff, and other staff as needed
  • To effective liaise with other health care team members
  • Satisfy continuing competence requirements as defined by expectations from the Ontario College of Pharmacists, Canadian Society of Hospital Pharmacists, and the Pharmacy Department
  • Performs crossfunctional and/or other duties consistent with the job classification, as assigned or requested
  • Understand and use available technology appropriately, as deemed necessary by specific responsibilities
