- Work collaboratively with the Pharmacy Team to provide patient-centred care
- Provide Pharmaceutical care and be part of a multi -disciplinary team, including medication reconciliation upon admission and discharge, therapeutic drug monitoring
- Provide drug information to physicians, nurses, and other clinicians to enhance patient care
- Verification of all medication orders for inpatients
- Provide education to patients to promote optimal drug use
- Mentor Pharmacy residents, students and interns
- Participate on hospital committees as required
- Dispensary duties including medication order entry and medication profile review
- Perform and/or supervise in patient dispensing (which includes computerized order entry, unit dose, centralized IV admixture services, TPN, Chemotherapy and an understanding of automated systems) drug information and administrative activities as assigned
- Resolve day-to-day clinical issues
- Assist with the implementation of pharmacy initiatives, which improve patient care and other initiatives which improve the efficiency and effectiveness of the Pharmacy operations
- Other duties as assigned
- Currently licensed (or eligible) by the Ontario College of Pharmacists, Part A of the registry
- Relevant clinical experience preferable
- Completion of hospital pharmacy residency or equivalent experience an asset
- Excellent attendance, punctuality and work record
- Demonstrate initiative and a willingness to work collaboratively in a multidisciplinary team environment
- Motivated, self-directed learner
- Strong clinical assessment skills in identifying and resolving drug-related problems
- Demonstrated ability to apply clinical judgment in analyzing and solving clinical problems
- May be required to travel between and work at all Osler sites
- Excellent oral, interpersonal, and written skills
- Excellent computer skills
- Must be able to meet the physical demands of the position
