Client Care Coordinator


Are you a strong multitasker who likes to work in a fast-paced environment? If so, Provincial Homecare is actively hiring for a Client Care Coordinator in our CBS office.


Job Summary

Responsibilities

  • Notify clients and Home Support Workers regarding initial and ongoing schedules.
  • Communicate any schedule changes to those affected by the changes such as the client, caregiver and/or HSW.
  • Complete data entry and maintain accurate scheduling documentation in Alayacare.
  • Assist with training and mentoring of all Home Support Workers by directing them to our website training modules.
  • Ensuring Client profiles in Alayacare are accurate (noting required skills & nursing delegations)
  • Participate in client satisfaction initiatives.
  • Participate in ongoing internal and/or external continuing education activities.
  • Adhere to Provincial Homecare Policies and Procedures.
  • Maintain confidentiality of client and corporate information
  • Complete other administrative tasks as required

Qualifications:


If you're a team player who is eager to learn new skills, develop a relationship with both clients and home support workers then we want to meet you With a background in client satisfaction or schedule development, the successful applicant will be professional and able to adapt to changing client needs.

Previous Home Support Worker experience would be an asset

  • Ability to meet deadlines and strong commitment to continual learning;
  • Ability to demonstrate competency in scheduling software;
This position is Monday - Friday from 8:30 AM to 5:00 PM. Our Client Care Coordinators also have a rotational on-call period shared by the team.

  • Clarenville-Shoal Harbour, NL: reliably commute or plan to relocate before starting work (required)

Education:


  • Secondary School (preferred)

Experience:


  • Scheduling: 1 year (required)
