Care Coordinator

We are seeking an RPN to fill the role of Care Coordinator at our Memory Care Retirement Residence.

This position reports to the General Manager, and collaborates with the nursing staff to influence and advance the standard of clinical resident practice and care outcomes.


Responsibilities:


  • Participates with the resident care team to improve clinical care practices
  • Fosters effective interpersonal relationships between residents, families and other interdisciplinary health team members
  • Works in collaboration with registered staff in the planning, developing and evaluation of nursing practice
  • Accepts referrals from care team members and recommends referrals as necessary
  • Undertakes problem solving in complex client care problems/dilemmas/crises
  • Identifies, analyzes and works toward the resolution of client care problems across nursing (e.g. falls prevention, skin and wound care management, continence care management, pain management, bowel management and behaviour management).
  • Advocates for the resident and family to ensure that their rights are upheld, (e.g. access to resources, information, medical care and to understand choices in relation to treatment.)
  • Actively participates in quality management activities in the home
  • Ability to work the floor and interact with residents, providing care and administering medications
  • Provides leadership for evidence based practice
  • Develops implements and evaluates clinical care for selected resident based on clinical needs.
  • Identifies and analyzes trends in resident care outcomes and practice to clinical standards.
  • Acts as education resource to all members of the resident care team.

Qualifications:

  • Registered Practical Nurse with current certification of competence from the College of Nurses of Ontario
  • Current membership with weRPN is desireable.
  • Has at least 3 years of working in the retirement home sector
  • Has at least 3 years of registered practical nurse in a managerial or supervisory capacity in a health care setting and,
  • Has demonstrated leadership and communication skills
  • Experience working with PointClickCare an asset

Salary:
From $24.00 per hour

  • French not required
