Description

The Social Worker will possess a solid understanding of the challenges individuals and families encounter while living with chronic diseases.


Key Responsibilities:


  • Perform Psychological assessments, therapeutic intervention/counselling to individuals, and families.
  • Act as a resource to diabetes educators to provide assistance in addressing complex client needs.
  • Develop and implement support groups, workshops and educational opportunities as indicated by regular needs assessments of patients, hospital and community.
  • Advocate on behalf of individual and systemic issues as they arise.
  • Contribute to chart documentation, participate in chart reviews, and team meetings with patients as needed.
  • Assist with clinical support for students and volunteers as required.
  • Develop and provide group counselling services.
  • Assist patient to access financial aid and medication coverage.
  • Incorporate respected smoking cessation and behaviour change and techniques to assist patients reach their goals.
  • Engage in education and health promotion activities on and off site in conjunction with other service providers.
  • Performs duties in a manner that exemplifies and facilitates both quality and patient safety.
  • Identify the needs of individual clients and communities and tailor our services to best suit their needs.

  • Carry out duties in a way that embodies the mission, vision and values of providing a patient and familycentered experience and highquality care.
  • Work cooperatively with fellow employees and management towards achievement of the Hospitals' strategic priorities.
  • Perform other duties as assigned that are reasonable within the scope of practice.

Qualifications Required:


  • Bachelors of Social Work and/or a Master's degree.
  • Must be a member in good standing with the OCSWSSW.
  • Medical exposure is considered an asset.
  • Working experience with individuals with mental illness.
  • Experience in program development, implementation and evaluation.
  • 3 to 5 years current counselling/therapy experience within a hospital setting.
  • Excellent interpersonal and communication skills (verbal and written) and good judgement when relating to patients and diabetes/chronic illness, families, team members and interprofessional teams.
  • Ability to provide counsel and therapy to assist clients in developing selfmanagement skills for emotional challenges raised by diabetes.
  • Ability to serve as member on interprofessional teams.

Other pertinent conditions:


  • Salary as per the Nonunion salary schedule ($38.02 per hour to $49.29 per hour)
