As a member of the Tofield LTC team, the Health Care Aide will be a key part of interdisciplinary care team providing personal care and support to our residents.

You must demonstrate genuine interest and respect for the residents and utilize your knowledge to provide comfort and support services to residents.

Tofield LTC is a 50-bed facility and we work closely with the resident and their family members to establish a "patient centred" care plan.

The HCA will work under the guidance of a Registered Nurse, Licensed Practical Nurse and other health care professionals in accordance to the resident care plan, policies and procedures within AHS.


Description:


As a Health Care Aide (HCA), you are responsible for providing personal assistance, supporting activities of daily living and providing comfort and support services to patients (clients, residents) who require short-term assistance or ongoing support.

You will work closely with patients, families, and caregivers spanning the continuum of care in home, community or health care facilities.

You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Additional Required Qualifications:


Ongoing education to meet the CCHSS standards and AHS qualifications is mandatory, with documentation that such education requirements are being met must be submitted on a regular basis.

Must be organized and be able to set priorities to complete assigned work, along with working within a team setting.

Must be able to meet the physical demands of the position. Knowledge of IPC issues a must and are enforced at all times. Excellent attendance record required. Able to work shift work and weekends required. PC computer and experience in the use of Microsoft programs.


Preferred Qualifications:

Experience working with Connect Care.
