Duties:


  • Work with an adult population with a serious mental illness (schizophrenia and related disorders) and their families
  • Assessment, treatment, and support to clients and families
  • Complete risk assessment, crisis intervention plans, and clinical needs assessment
  • Provide education and treatment in individual and group settings for patients and families
  • Liaise with appropriate residential settings and community resources
  • Discharge planning, in collaboration with the clinical team and families
  • Advocate on behalf of clients and families for appropriate services and resources
  • Engage client, family, and significant others in the clinical interventions and risk reduction strategies with the interdisciplinary team
  • Develop and lead groups
  • Attend monthly Social Work discipline meetings
  • Attend and participate in monthly Social Work Peer Consultation Groups
  • Assist with planning and implementation of Community Treatment Orders
  • Demonstrate thorough knowledge of the theories and practices of a professional social worker
  • Document on patient's clinical record according to hospital, College, and legislative standards
  • Maintain work load measurement and data collection (e.g. RAI) as required
  • Participate and lead in research and program evaluation
  • Participate in supervision, teaching and education of Social Work students, and support students from health related disciplines
  • Participation in hospital Social Work and quality improvement requirements
  • Keep current of hospital policies and procedures and applicable legislation
  • Keep up to date regarding professional standards by reviewing current literature, attending educational events, networking with colleagues, etc.
  • Participate in Program and discipline specific committees and task groups
  • Provide educational presentations to community groups
  • Work in a manner that is in compliance with staff and patient/client safety practices, policies and procedures of the Royal
  • Ensure a work environment that is conducive to The Royal's Anti-Racism, Discrimination & Harassment-Free Workplace Policy

Qualifications:


  • Masters degree in Social Work
  • Registration and in good standing with the Ontario College of Social Workers and Social Service Workers
  • Minimum of three (3) years clinical experience in a mental health setting
  • Discharge planning experience and knowledge of community resources is essential
  • High level of knowledge, skill and confidence in using electronic health record systems for clinical documentation and navigation.
  • Experience in group work preferred
  • Knowledge of Cognitive and / or Dialectical Behavioural Therapy is preferred
  • Client centered approach
  • Experience with recoverybased programs for the seriously mentally ill is an asset
  • Experience working with Concurrent Disorders population asset
  • Conflict resolution knowledge
  • Ability to work as a member of an interdisciplinary team
  • Valid drivers license and access to a vehicle
  • Willingness to work variable hours, evenings and weekends
  • English level A is mandatory in oral expression, oral comprehension, reading comprehension and written expression. Bilingual (French/English) is considered an asset

Salary Range:

- $40.80 to $51.98 per hour
