Duties:

Work with an adult population with a serious mental illness (schizophrenia and related disorders) and their families
Assessment, treatment, and support to clients and families
Complete risk assessment, crisis intervention plans, and clinical needs assessment
Provide education and treatment in individual and group settings for patients and families
Liaise with appropriate residential settings and community resources
Discharge planning, in collaboration with the clinical team and families
Advocate on behalf of clients and families for appropriate services and resources
Engage client, family, and significant others in the clinical interventions and risk reduction strategies with the interdisciplinary team
Develop and lead groups
Attend monthly Social Work discipline meetings
Attend and participate in monthly Social Work Peer Consultation Groups
Assist with planning and implementation of Community Treatment Orders
Demonstrate thorough knowledge of the theories and practices of a professional social worker
Document on patient's clinical record according to hospital, College, and legislative standards
Maintain work load measurement and data collection (e.g. RAI) as required
Participate and lead in research and program evaluation
Participate in supervision, teaching and education of Social Work students, and support students from health related disciplines
Participation in hospital Social Work and quality improvement requirements
Keep current of hospital policies and procedures and applicable legislation
Keep up to date regarding professional standards by reviewing current literature, attending educational events, networking with colleagues, etc.
Participate in Program and discipline specific committees and task groups
Provide educational presentations to community groups
Work in a manner that is in compliance with staff and patient/client safety practices, policies and procedures of the Royal

Qualifications:

Masters degree in Social Work
Registration and in good standing with the Ontario College of Social Workers and Social Service Workers
Minimum of three (3) years clinical experience in a mental health setting
Discharge planning experience and knowledge of community resources is essential
High level of knowledge, skill and confidence in using electronic health record systems for clinical documentation and navigation.
Experience in group work preferred
Knowledge of Cognitive and / or Dialectical Behavioural Therapy is preferred
Client centered approach
Experience with recoverybased programs for the seriously mentally ill is an asset
Experience working with Concurrent Disorders population asset
Conflict resolution knowledge
Ability to work as a member of an interdisciplinary team
Valid drivers license and access to a vehicle
Willingness to work variable hours, evenings and weekends
English level A is mandatory in oral expression, oral comprehension, reading comprehension and written expression. Bilingual (French/English) is considered an asset

Salary Range:

- $40.80 to $51.98 per hour
