CBI Health is looking for a dynamic and compassionate nurse leader to be the Weekend Transition Lead in an innovative and collaborative Southlake Hospital transitional program that includes working with an interdisciplinary team that uses an integrated and collaborative approach to service delivery.

CBI Health has strengthened the traditional In-Home service delivery model focusing on a holistic approach to care.As the primary contact for a hospital partner, the Weekend Transitional Lead will manage referrals and client intake while working with hospital lead, patients and families, and other community partners, to create individualized and holistic plans of care to support patients to transition home with wrap around services and remain at home.
Your role is vital in coordinating smooth discharges from hospital to home, working with the circle of care to ensure the progress of patient care plans and identifying solutions and innovative approaches to meet patient needs.
As the Weekend Transitional Lead you will be the primary contact to front line providers, community partners and the after hours team, to aid in the resolution of any concerns related to client wellness and care.
The Transition Lead may participate in interdisciplinary discussions (huddles, rounds), and lead discharge and transition planning meetings that will result in smooth patient transitions.

Registered with the College of Nurses of Ontario in good standing (RN or RPN) or Registered with the College of Physiotherapists in good standing or Occupational Therapist registered with the College of Occupational Therapists in good standing
Minimum of 2-3 years of nursing/PT/OT experience
Experience in home and community care or acute care (community experience is considered an asset)
Excellent written and verbal communication skills
High degree of professionalism and patient/family focused care
Be a team player and be highly collaborative
Strong computer literacy with various applications and programs
Demonstrated clinical leadership & ability to lead discussions and collaboration
Excellent assessment and coordination skills
Demonstrated ability to work within an interdisciplinary team
Strong organization skills and able to prioritize/ multi-task in a fast-paced environment
Access to a reliable and insured vehicle
A valid driver's license and ability/willingness to travel within York Region
Ability to communicate in French or other languages is considered an asset
