- Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client's unique needs
- When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
- Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

- Availability to workdays, evenings, nights and every other weekend.

- This is a physically demanding position and requires hand, arm and shoulder dexterity, sensory ability, lifting to a maximum of approximately fifty-five (55) pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement.

We are seeking several dynamic, passionate full and part-time (days, evening, night shifts) Registered Nurses (RNs) for Newmarket, Bradford, Holland Landing, Mt Albert and Schomberg areas, who want to be part of the transformational change in-home and community care by providing care to pediatric clients in a truly client-centered approach. You will be part of a diverse and collaborative care team that will provide services to a roster of clients in the community.
