Patient Care Coordinator to join our clinic. This is an opportunity to be the expert manager for a specialized service and if you have a passion for delivering impeccable customer service, supporting patients through their care trajectory, and adept at managing administrative duties, then this role is for you

This is a Full Time permanent, position, on-site, weekdays (5 days per week).


The
Patient Care Coordinator is the main point of contact for the clinic and serves as the liaison between the medical teams and patients.

Their main responsibility is to provide outstanding customer service, schedule appointments, manage administrative duties, and maintain accurate, confidential patient records.

The role requires exceptional communication skills, attention to detail, empathy, good judgment, the ability to work independently and to handle multiple tasks in a fast-paced environment.


Main responsibilities:


  • Greet patients in a friendly, professional manner
  • Establish and maintain confidential electronic medical records (EMR), in compliance with all relevant health regulations (ie. HIPAA), to ensure patient privacy and data security
  • Schedule appointments in the scheduling system, ensuring optimal utilization of the physicians' time and reschedule appointments as necessary
  • Provide all necessary preparation instructions to patients
  • Manage patient care trajectory through clinic experience from initial appointment, visit, all followups, and all ongoing communications
  • Maintain and update digital patient records, ensuring accuracy and confidentiality
  • Build relationships and coordinate with other health care stakeholders (other physicians, nurses, hospital staff) to ensure optimal patient flow.
  • Guide patients to complete insurance forms and provide letters, offwork notes to patients
  • Collect payments, outstanding balances and provide relevant documentation.
  • Manage electronic and paperbased documents : digitize, file, and manage all records in an efficient and accurate manner.
  • Assist in managing office supplies, equipment and ensure the working area is clean, organized and presentable.

  • Proficient in using MS Office, Apple environment
  • Familiarity with the electronic medical record (EMR)
  • MYLE/Medfar an asset
  • Knowledge of medical terminology desirable
  • Previous experience will be considered an asset
  • High school diploma or equivalent
  • An advanced degree /diploma (Bachelors, diploma AEC, DEP, DES) are strong assets

  • Planifier les rendezvous dans le système de planification en assurant une utilisation optimale du temps des médecins et recéduler les rendezvous si nécessaire
  • Répondre au téléphone et aux courriels de la clinique de manière courtoise et professionnelle pour assurer un service à la clientèle impeccable dans la gestion des préoccupations et des demandes des patients et fournir des suivis précis et opportuns
