Education: Completion of a recognized Social Service Worker or Social Service Diploma Program or an equivalent combination of education, training and experience.

Duties: Collaborates with Social Work to follow


Assists in the development of resource materials for patients/clients/families such as Home Health Services, Community Services, Financial Resources and Mental Health and Substance Abuse resources.

Participates in activities that facilitate patients/clients goal setting in an inter-professional and collaborative manner, and is able to effectively problem solve, and resolve conflict.

Completes required reports and maintains update patients/clients records on assistance provided and resources applied in accordance with established procedures for documentation and other organizational reporting purposes.

Provides work direction to college practicum students from Social Service Worker diploma programs.
Carries out responsibilities in accordance with health and safety requirements.
Performs other related duties as assigned.
