About the Opportunity:

The Health Care Social Worker provides a diverse range of interventions to clients and their families, enabling them to achieve their optimal level of functioning and health


These interventions include:
psychosocial assessments, adjustment therapy, supportive therapy, crisis counselling, resource counselling, and discharge planning and case management.

Health Care Social Workers are actively involved in advocacy, education, program planning, consultation and collaboration on behalf of individuals/groups within the institution and community.

The role is multifaceted and dynamic, requiring each Social Worker to work autonomously as well as collaboratively within interdisciplinary teams.

The Health Care Social Worker is a licensed professional who practices in accordance with a set of National/Provincial Standards and Code of Ethics and Department Procedures, Policies and Standards.

The Health Care Social Worker is responsible for supervising undergraduate and graduate students in work placements.

The Health Care Social Worker provides consultation based upon an advanced level of education (MSW) which includes an integrated knowledge of the social, structural, emotional, psychological and cultural factors tha1 affect well-being.


The Social Worker will be part of the interdisciplinary MAID Access and Resource Team, with a specific focus on addressing psychosocial needs of individuals referred to the MAID team and their support system.

The Social Worker will be responsible for providing virtual assessments and developing care plans that address identified psychosocial needs which may include anticipatory grief, legacy planning, supportive counselling, resource navigation as well as post-MAID support for family/caregivers for debriefing and grief support.

The Social Worker will also support the development of patient/family resources, staff education, support staff in navigating and debriefing complex cases related to psychosocial issues, and work collaboratively to promote provider wellness and resiliency.

This position is located in Central Zone; the incumbent will be required to work in this zone.


About You:

We would love to hear from you if you have the following:

  • Completion of a Master of Social Work degree from a recognized university
  • Registration and Membership in the Nova Scotia Association of Social Workers and Canadian Association of Social Workers
  • Two years clinical experience as a Social Worker required
  • Knowledge of the fundamentals of human behaviour including systems theory, family dynamics, group dynamics, and theories of behaviour change
  • Experience in the areas grief and loss, end of life, disability, crisis intervention, caregiving and continuum of care and intervention skills
  • Awareness of local community resources and agencies would be an asset
  • LEAP palliative care training preferred
  • Proven selfdirected leadership
  • Competencies in other languages preferred, French an asset

  • Long Assignment Fulltime Position; 75 hours biweekly
  • Approximately 18 months

Compensation and Incentives:

$ $48.81 Hourly
