Position Overview:


  • Establishes collaborative working relationships with both physicians and health care teams within the hospital as well as in the community, and with patients and family members to initiate, implement and monitor community treatment orders
  • Facilitates rights advice and acts as a resource for the health care team in preparing for Consent and Capacity Board hearings
  • Completes all documentation as required by legislation, MOHLTC and Trillium Health Partners

Qualifications:


  • Bachelors Degree or Community College degree in a health related discipline
  • Registration with a recognized college
  • Minimum of 3 years of postgraduate experience in Mental Health; preferably in the community and hospital setting
  • Thorough knowledge of the Ontario Mental Health Act and Health Care Consent Act
  • Excellent communication and organizational skills
  • Excellent collaboration skills with patients, families and other healthcare professionals
  • Exceptional problemsolving and decisionmaking skills
  • Demonstrated ability to work independently and as a member of an interdisciplinary team
  • Knowledge of community resources
  • Valid driver's license; relevant insurance and use of own vehicle
  • Satisfactory performance and attendance required
  • Proficiency with computer skills
