What We Need:

Responsibilities (including but are not limited)

  • Reviews incoming incident reports for completion and accuracy and flags for follow up
  • Conducts and/or support quality of care reviews, incident analysis, Root Cause Analysis/ Failure Mode and Effects Analysis, including tracking of recommendations implementation.
  • Review of patient safety data to identify trends and actionable items to advance a just culture
  • Assist in the preparation of reports in the form of tracking, trending, graphs, or analyses regarding patient safety program outcomes
  • Facilitates the timely investigation of select safety events and near miss events through the implementation of cause analysis and use of other investigative tools
  • Establishes and maintains effective relationships with staff and physicians in order to influence and advance safe practices and behaviors
  • Serves as a resource to managers on effective corrective action design to minimize event recurrence
  • Support and participate in the development, implementation, and evaluation of patient safety improvement initiatives
  • Promotes a "just culture" environment, consults with and provides advice to organizational leaders regarding error prevention strategies
  • Serve as a patient safety resource, advisor and support for leadership related to investigations/procedures/interviews
  • Serve as a resource to all program/services/departments including coordinating patient safety program, educating employees about patient safety management policies and procedures, etc.
  • Liaises with the Patient Relations Specialist, as required, in the management of patient complaints/patient safety issues
  • Supports program/services/departments in the compliance and evaluation of Accreditation standards.

What you bring to the role:

  • A degree in a health care related field or equivalent acute care (hospital) experience
  • Canadian Patient Safety Officer (CPSO) certification or equivalent considered an asset
  • Knowledge of RL6 solutions and Meditech preferred
  • Recent and related experience in quality and safety in a hospital setting
  • Demonstrated ability to utilize quality improvement tools and techniques
  • Ability to review and analyze data and ability to generate written and verbal reports
  • Demonstrated computer software expertise including, spreadsheets, graphics, PowerPoint
  • Selfdirected and strong collaborative skills
  • A strong understanding and knowledge of the hospital health care system and health legislation including the Excellent Care for All Act (ECFAA), Freedom of Information and Privacy Protection Act (FIPPA), Personal Health Information Protection Act (PHIPA), etc.
  • Excellent interpersonal and communication skills with demonstrated ability to develop trusting, credible relationships with all levels of staff, physicians, volunteer
  • Excellent problem solving, decisionmaking, critical thinking skills and ability to generate ideas and fresh perspective
  • Ability to deal with multiple complex issues and manage multiple priorities
  • Strong mediation and conflict resolution skills and ability to remain thirdparty neutral
  • Demonstrated good attendance and performance records with the ability to maintain these same standards
  • Demonstrated commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required

Work Schedule:

Primarily days - Monday to Friday with flexibility to meet needs of the department. Schedules may change based on operational needs.


Compensation

BAND H:
$ $60.59 per hour

Annual Salary:
$82, $118,150.50 per year
