Overview

Reporting to the Director of Care, the Social Worker is responsible for the development and implementation of the Social Work Program at Miramichi Lodge.

The goal of the Social Work Program is to ensure that residents are provided with a safe, person centred, and strengths-based plan of care consistent with Ministry of Health/Ministry of Long-Term Care standards, and compliant with the professional accountabilities of the Canadian Association of Social Workers and the Ontario College of Social Workers and Social Service Workers (OCSWSSW).

Social work services at Miramichi Lodge are designed to meet the biopsychosocial needs of residents, their families, and others involved with the residents' care, and are delivered in a manner that ensures confidentiality and cultural competence in accordance with professional standards.

Social services include, but are not limited to; spiritual programs, palliative care support, individual or family therapy, emotional and behavioural support, wellness promotion, and connections to community resources.


Responsibilities:


  • Performs the responsibilities of the position within the legislative and regulatory standards set out in the Canadian Association of Social Workers and the Ontario College of Social Workers and Social Service Workers.
  • Performs the responsibilities of the position consistent with the policies and procedures of Miramichi Lodge and the County of Renfrew.
  • Ensures that Social Work programs and services are provided in a manner that focuses on the satisfaction of the residents and fully supports their rights, in collaboration with other members of the interdisciplinary team, inclusive of supporting capacity assessments, and resident advocacy.
  • Provides social services delivered in the context of multicultural understanding and competence, in an environment that fosters diversity, equity, and inclusion (DEI).
  • Supports the Home in achieving and maintaining a therapeutic environment essential to the optimization of the quality of life and independent functioning of each resident.
  • Promotes community engagement and interaction by encouraging community involvement in the Home, along with resident and staff involvement in the community. Developing relationships and seeking out opportunities for collaboration with a wide range of community resources, and reports on identified strengths as well as opportunities for improvement in both internal and external senior support services.
  • Provide specialized support to those living with dementia, including addressing the behavioural and psychological symptoms of dementia, acting as a resource within the interdisciplinary team, and participating in behavioural interventions.
  • Accountable for the implementation and oversight of policies and procedures to ensure adequate documentation of social work services is included in the resident's medical record, and that legal, ethical, and professional standards are observed in documentation practices.

Qualifications:


  • Successful completion of a Bachelor's Degree in Social Work as recognized by the Ontario College of Social Workers and Social Service Workers
  • Member of the Ontario College of Social Workers and Social Service Workers, in good standing.
  • Three (3) to five (5) years relevant experience in a health care setting, preferably working in a communal care setting, longterm care experience preferred.
  • Familiarity of the biopsychosocial needs of the longterm care population, knowledge of gerontology, Alzheimer's disease and related dementias.
  • Sound understanding of relevant legislation, including the LTCHA, Health Care Consent Act, Consent to Treatment Act, Substitute Decisions Act, and PHIPA.
  • Current knowledge related to the Consent and Capacity Board and Powers of Attorney.
  • Knowledge of available community programs and resources, including financial assistance and other community support services aimed at meeting the needs of older adults.
  • Demonstrated skills in client/family assessment, intervention, advocacy, and knowledge of human behaviour and counselling techniques reflective of current best practices.
  • Experience in case management, service planning, and the development, implementation, and evaluation of a strengthsbased plan of care.
  • Demonstrated commitment to fostering a culture of diversity, equity, and inclusion.
  • Excellent interpersonal, verbal and written communication skills. Experience working with electronic health records is an asset.
  • Computer literacy required (word processing, spreadsheet and database), with demonstrated ability to compile statistical data and reports for review and evaluation.
  • Ability to meet and maintain health and safety standards in accordance with applicable legislation(s).
  • Flexibility to work a variety of hours as required to accommodate resident and organizational needs.

Job Types:
Full-time, Fixed term contract

Contract length: 12 months


Salary:
$40.21-$45.26 per h
