Background and Job Description
The Regional Geriatric Program of Toronto (RGP) supports a network of healthcare providers who care for frail older adults.

Building capacity for senior friendly care through education and training is a central pillar of the RGP mandate and strategic plan.

We develop and implement innovative and complex knowledge-to-practice (KTP) initiatives. Varying in scale, these initiatives are inter-professional, inter-sectoral and co-designed with key stakeholders, including older people themselves.


The RGP of Toronto is seeking an experienced health professional with interest in the care of older people, continuing health professional development and knowledge-to-practice processes.

Working closely with the RGP directors, the Knowledge Translation and Implementation Project Manager will be responsible for the design, implementation and evaluation of a variety of KTP initiatives, ensuring timely progress and achievement of expected deliverables.

The Knowledge Translation and Implementation Project Manager will serve as a leader, coach, and expert in educational program development and in implementing knowledge translation strategies.


Responsibilities:


  • Develop learning resources based on evidence and best practices
  • Develop strategies for knowledge transfer and evaluate its impact
  • Create implementation plans for complex knowledgetopractice interventions and ensure timelines and deliverables are met
  • Serve as an information broker on topics relevant to geriatric care, including evidencebased interprofessional geriatric clinical care and service development
  • Contribute to innovation in education
  • Provide coaching and support the implementation and spread of evidencedbased interventions
  • Evaluate the impact of KTP activities by collecting and analyzing data; prepare reports, presentations summarizing the impact of the KTP activities of the RGP of Toronto
  • Develop and implement knowledge exchange opportunities that are innovative, interprofessional, intersectoral and multimodal
  • Collaborate with other RGPs in Ontario on educational initiatives
  • Collaborate with all stakeholders in the development and implementation of key projects

Qualifications:


  • Master's degree in healthcare field, health administration, implementation science, engineering or related discipline is required
  • Regulated Health Professional in good standing an asset
  • Minimum 3 to 5 years applicable experience working in the healthcare sector with a focus on Knowledge Translation and/or Quality Improvement
  • Experience in the core geriatric competencies and the knowledgetopractice process an asset
  • Demonstrated knowledge and experience with project management processes, principles and tools; Project Management Professional (PMP) designation preferred
  • Knowledge of the principles of implementation science and continuing health professional development
  • Strong leadership skill set, including but not limited to building relationships and change management, working through influence, negotiation, collaborative problem solving, developing and implementing strategy.
  • Demonstrated ability in leadership, coaching, mentoring, small group and team facilitation
  • Understanding of the healthcare system and the unique challenges of geriatric care
  • Experience with literature review processes and knowledge synthesis
  • Proficiency in Microsoft Office (i.e. Excel, Word, Powerpoint etc.) and data management tools
  • Highly effective presentation and meeting facilitation skills including the ability to present and discuss complex information in a way that establishes rapport, influences others and enables understanding
  • Proven ability to work independently, take initiative and perform in stressful environments
  • Knowledge in determining local facilitators and barriers to implementation and developing plans to leverage or mitigation them
  • Able to meet deadlines and manage competing priorities
  • Strong commitment to fiduciary responsibilities
  • Satisfactory performance and attendance required

Accountability


The Knowledge Translation and Implementation Project Manager works under the supervision of the RGP Directors and collaborates with program directors, managers, health professionals, and other staff throughout the RGP Network.
