JOB SUMMARY:

This role is Temp until August, 2023


The clinical practice lead is responsible for leading the education initiatives for all team members within long term care and support areas.

Through the fulfillment of our Mission, Vision, and Values, the clinical practice lead enhances team member performance through education, coaching, mentoring and guidance.

The clinical practice lead establishes and maintains relationships that exemplify servant leadership principles with team members, residents, families and external agencies.

The clinical practice lead is responsible for planning, delivering and evaluating educational programs to the team members.

Within the Wellness Model of care and service, the Practice Leader supports and respects the autonomy, independence, dignity and privacy of our residents and fosters a positive work environment for team members.

This contributes to the safety of both residents and team members.

The clinical practice lead values learning, embraces innovative change and promotes health and wellness using a person-centered/resident directed and normalized living approach.

  • Works closely with the team members as a mentor to ensure ongoing leadership development.
  • Models the multiskilling concept.
  • Supports the team members through the accreditation process including selfevaluation; ensuring team members are aware of policies, procedures, goals and clinical practice requirements.
  • Program Leadership_
  • Leads the development, implementation, evaluation and delivery of an Orientation Program and Clinical Practice Program for team members, residents, and family.
  • Works with the Baptist Housing education team to sustain and enhance established standardized programs and practices.
  • Acts as a resource for all team members with regards to selfidentified educational needs, trends in gerontological care and service, best practice, etc.
  • Provides educational opportunities for "champions" for various programs to support their knowledge and development.
  • Consults with educators and clinical practice leaders regarding identified best practice areas requiring education and individuals who require further educational assistance.
  • Ensures all education programs respond to the individual resident's physical, emotional, intellectual, spiritual, and social needs.
  • Ensures standards of practice comply with professional, legal, and organizational requirements.
  • Fosters and maintains positive, collaborative relationships with clinical and management teams, support services departments, physicians, other health professionals and community agencies.
  • Participates in committees that relate to quality management and contributes to the implementation of quality management programs.
  • Maintains an updated gerontological resource library and monitors the usage and location of library resources.
  • Advocates for improvements/advancements in practice and gerontological healthcare.
  • Reporting_
  • Ensures that the RAI / MDS reporting competency requirements are met annually.
  • Provides a monthly report to the senior clinical director.
  • Maintains appropriate records of educational activity attendance, including fire drill attendance, annual testing, inservices, orientation, etc.
  • Networking and Relationships_
  • Participates as a member of the clinical services team (CST), which meets quarterly to communicate, plan, and review education needs and strategy.
  • Partners with human resources in the development and/or implementation new team member orientation
  • Creates positive, collaborative partnerships with key partners, including:
  • Health Authority
  • Support Services Contract Providers
  • Educational Institutions (including research programs)
  • Gerontological Nurses Association
