Peel YMCA Employment and Community Services is part of YMCA Employment and Community Programs which provide a variety of employment and counselling programs to youth and adults throughout the GTA.


The mandate of the Life Stabilization program focuses on enabling individuals to gain self-sufficiency, address preparatory and/or urgent needs through referrals to health, legal, crisis response, social supports, family support and other human services.

These services and supports may precede or be provided concurrently with employment training activities, depending on an individual's circumstances and capacities.


The Case Manager supports individuals, people with disabilities, newcomers, Indigenous, and young people with higher needs succeed in retaining sustainable employment.

By following a collaborative and client-centered process, this position supports the timely access to the right services and supports to help a member achieve their employment goals and monitors their progress in achieving those goals.

It may include identifying further needs and supporting access to community-based services that may affect the member's readiness for employment, such as income support, family support, transportation and health care services.

The intensity and duration of case management will vary depending on the individual's needs, which may extend beyond a job placement to job retention supports and services.

Case Management will support Indigenous clients to receive culturally appropriate service.


In this role, you will:


  • Contribute to monthly individual and team Key Performance Indicators (KPIs) while managing other tasks through prioritization and effective time management.
  • Using the Common Assessment tool, develops appropriate Employment Action Plan with member based on assessment.
  • Develop strategies and timelines for individual members for achieving goals while problemsolving when obstacles or challenges occur.
  • Conduct casemanagement including needs and risk assessment, contracting, implementation and followups as it pertains to goals and other needs. Utilize consistent intake tool and approach to evaluate individuals' Employment Service and Life Stabilization service needs; identify and assess members' need for job, life skills, etc. coaching.
  • Connect with other services and resources as required, brokering services wherever relevant; seeks to create and/or maintain partnerships with relevant service agencies through community outreach
  • Utilize electronic tracking system to record and report on the services being offered to individuals.
  • Provide advocacy support to members with personal, systemic, and/or environmental barriers (e.g. addictions, chronic disease, homelessness, mental health).
  • Prepare case management and case note documentation and handles confidential data; ensures quality audit measures are achieved in accordance with the YMCA and funder's standards.
  • Input, maintain and update member information and outcomes in a timely manner into database.
  • Advocate on behalf of the member to community employers, services and programs that will move them closer to their employment and/or other goals and provide coordinated service. Consult with team members and community partners to find employment opportunities, and monitor progress in the work environment. Participate in outreach events/ activities through collaboration and relationship development with community partners.
  • Ensure selfmanagement of program targets related to the employment program delivery and funder expectations; complete and maintain member files as per funder and YMCA quality standards.
  • Perform other duties as assigned.

You bring:


  • Post-Secondary degree/diploma in Career Planning, Employment Counselling, Social/Community Work, Social Services, Human Resources or related field or equivalent training/experience
  • Minimum two years' experience in counselling, file management, group facilitation, or job coaching is preferred
  • Knowledge of issues that prevent individuals from being employed, and an ability to work with members in a respectful and nonjudgment and consistent manner
  • Knowledge of the philosophy of Harm Reduction, substance use, or mental health and/or concurrent concerns
  • An ability to develop and maintain strong professional relationships with employers/business, community partners, and colleagues for the purpose of coordinating case management
  • Access to a motor vehicle is required for this position, as extensive travel within the region is required
  • Superior presentation and writ
