Position Overview:


Under the direction of the Adult Transitional Housing Services Manager, the Back to Home case manager is responsible for the continuous implementation of case management to clients accessing an Alternate level of care (ALC) in addition to the coordination and delivery of cultural programming to Back to Home clients.

This includes ensuring regular, appropriate documentation on all clinical files; regular conferencing/reviews of relevant cases; evaluating system effectiveness and recommending necessary change.

The case manager will support the current development, treatment, cultural and psycho-social needs of adults in the program in collaboration with other community partners.

The primary responsibility is to ensure the provision of case management services focusing on cultural supports for adults that is consistent with the philosophy, policies and standards established by Dilico Anishinabek Family Care.


Qualifications and Requirements:


  • HBSW or preference in area of Native Studies or related Human Services diploma with a minimum 3 years experience in Cultural programming for children and their families. A combination of skills, education and life learning specific to traditional knowledge may be considered
  • An individual who is of Anishinabe ancestry with understanding, practice and sharing of Anishinabe history, culture, traditions, ceremonies and values is preferred
  • A minimum three (3) years case management experience with adults in the field of addictions and mental health is required
  • Extensive experience, training and knowledge of Anishinabek culture and language
  • Satisfactory Criminal Records Search; including a Vulnerable Sector Search;
  • A valid Standard First Aid/CPR Certificate is required
  • Work flexible hours according to the needs of clients
  • May have some travel to the First Nations communities in and around the District of Thunder Bay
  • Work in the homes and locations familiar to families
  • Posses a valid Class "G" Driver's Licence, and
i. have access to a personal vehicle;
ii. carry satisfactory liability insurance;
iii. produce a satisfactory driver's record;

Hours of Work: 32.5 hours per week


Wage Range:
$ $37.25 per hour
