Excel Template Developer – Healthcare Submission Form with Rate Automation

Job description
I run a nurse staffing agency and need an Excel submission form redesigned to mirror my existing billing invoice layout.
I will provide:
- Current billing template
- Current nurse submission form
- Assessment types and rate structure per nurse
The new submission template must match the billing layout exactly so data can be copied directly into billing without reformatting.
STRUCTURE REQUIREMENTS:
The form must include the following columns in the same order as my billing sheet:
- Medicaid #
- Member Last Name
- Member First Name
- DOB
- RN Assigned
- Visit Date
- Type of Assessment (RA, CIC, CMCA, IA, NIYA, etc.)
- County
- Comments
RATE AUTOMATION (Per Nurse):
Each nurse has different rates depending on assessment type.
The template must:
- Auto-populate rate based on assessment type selected
- Store rates in a hidden/locked rate table
- Prevent editing of rate cells
- Auto-calculate payout per case
- Auto-sum monthly total at bottom
DATA VALIDATION REQUIREMENTS:
- Medicaid number required (cannot be blank)
- Duplicate Medicaid numbers flagged automatically
- Required fields highlighted if missing (Member Name, DOS, Type)
- Dropdown list for assessment type
- Sheet protection so nurses can only edit input cells
- Locked formula cells
DELIVERABLES:
- Final locked master template
- One sample nurse version populated with sample rate structure
- Brief explanation of how to duplicate per nurse monthly
Budget:
$350
Timeline: 3–5 days
Experience with healthcare billing templates preferred.
Contract duration of 1 to 3 months.
Mandatory skills:
Microsoft Excel, Automation, Accounting
