We are seeking a dynamic, highly motivated
Intensive Case Manager who demonstrates our values of compassion, accountability, responsiveness, and empowerment; to our clients, families, and each other.


The
Intensive Case Manager reports to the Manager of Halton Housing Services, this position will assist in providing intake supports and carrying out day-to-day administrative services.

Wesley Halton Housing offers services to the homeless in the Halton Region, supporting homeless families and individuals in a transitional program on-site.

Services are based on a client-centred, residential harm reduction, housing-first approach to service delivery.

This position is a
unionized, full-time, permanent position. The rate of pay is
$22.98 per hour.

**Key Responsibilities: - Works with assigned families and individuals in a housing program which works towards a housing plan. This includes maximizing income, filing taxes, obtaining ID, and connecting with community supports.

  • Responds to calls and referrals from community agencies in accordance with case load.
  • Places individuals and families into permanent leased units, securing appropriate housing based on individual needs.
  • Assesses and screens participant's/families' needs for services to support their transition to stable housing and optimize their capacity to sustain housing using an acuity tool and other resources.
  • Assess clients for any recent, current, or ongoing experience with domestic violence and make referrals to appropriate programs.
  • Assist participants with their move into independent accommodations and provide neighborhood orientation.
  • Maintain regular, effective communication with social housing providers, administrators, community partners, and private landlords, to ensure an integrated approach that will provide the best support possible for clients to maintain housing.
  • Report to the appropriate child welfare agency (CAS) if child abuse and/or neglect is suspected or witnessed.
  • Conducts regular visits with participants in their homes or shelter to provide supports specific to life skills, daily living skills, conflict resolution, crisis intervention and prevention, budgeting assistance, financial literacy, community integration and community building, grocery shopping, cleaning, etc.
  • Maintain appropriate and accurate recordkeeping practice, as per expectations of Wesley.
  • Supports approximately 1 shift per month of 24hour intake; flex time is available for additional hours worked.
  • Other duties as assigned.

What you will bring:


  • Postsecondary degree/diploma in Human Services or related field.
  • Registration and member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required.
  • Experience working with adults and families with mental health and addictions issues and homelessness while providing nonjudgmental supports to them.
  • Must be able to work flexible shifts including day, evening, and weekend hours as well as 24hour intake responsibilities.
  • Must be able to meet travel expectations of the position; a valid Driver's license and access to a vehicle is a definite asset.
