Care Coordinator - Transitions Program

The Role

Key Responsibilities

  • Facilitating and communicating program information to youth, families, and referral agents in an empathetic and detailed manner.
  • Developing a strong working knowledge of program content; responsive to youth, family and referral agent questions and concerns.
  • Ability to remain calm and neutral when presented with multiple demands and highly stressed individuals.
  • Provides information and education to youth and families regarding the withdrawal and stabilization process, adolescent substance use, relapse process, relapse prevention and harm reduction.
  • Provides direct support and counselling for youth and their families regarding substance use and treatment, may include crisis intervention and family support when required.
  • Develops and reviews comprehensive care plans with youth.
  • Responsible for supporting youth to complete action items in their care plan for the development of life skills, such as substance use strategies, community connections, finances, scheduling and attending appointments (medical, educational, legal and family/supportoriented visits).
  • Identifies and facilitates exit plans, develops relapse prevention plans, and coordinates the implementation of aftercare post discharge.

Required

  • Undergraduate degree in Social Sciences, Health or Social Work or Child and Youth Care with related experience of two 2 years;
  • Knowledge and experience with the withdrawal management/detoxification process for youth.
  • Experience working with highrisk youth in a counselling or other helping capacity.
  • Experience working with, and extensive knowledge of mental health and substance use issues.
  • Comprehensive understanding of motivational interviewing, harm reduction, strengthbased approach, clientcentred care, trauma informed practice and evidencebased treatment.
  • Demonstrates a level of cultural sensitivity and understanding of the client population's cultural and socioeconomic characteristics.
  • A valid BC driver's license, acceptance of source deductions and where applicable, health benefit coverage.
  • A criminal record check that passes the Criminal Records Review Act is mandatory.
  • Level 1 First Aid and CPR and related experience.
  • Covid vaccination as per Island Health directive.
